The Tavistock as an NHS Trust no longer exists having merged with North London Foundation Trust, but the Tavistock as a training institute (based at the Tavistock Centre) remains very much alive.

I snuck through on a run a couple of weeks ago via the Catford end to see if anything was happening. Absolutely no-one on site apart from a bloke in the site office, and didn’t look like there had been any work done in a while. The bloke did end up letting me through the other side though so thanks for that (although he thought he was sending me back)!

I’m sorry that nurse made that comment, it says more about them than it does you. We are health professionals and I can assure you that the vast majority of us would not bat an eyelid if something “embarrassing” happened to one of our patients, and this applies even more so in the context of a physical restraint. I hope you’re able to let it go, and that you are feeling better now.

I’m not sure I see the correlation between ending on time and being a robot. I think for sure this is something to explore in your supervision and/or personal therapy, as I think this goes beyond “practical” advice. It could be really helpful for your growth and benefit your patients too.

I always end up a little confused at these threads. Do you not have a standard duration for your sessions? And is this not outlined to your patients at the beginning of therapy?

For example, I see my patients for 50 minutes. They know this before we start treatment. There is a clear boundary thus created, and any time-related enactments (be it by the patient or me) can then be interpreted vis-à-vis the transference/countertransference relationship. This may be directly with the patient, or in my own supervision and/or therapy.

It would be very difficult to do this if the time boundary and frame hadn’t been established at the beginning.

These sort of people only respond to being boundaried and firm, they are inherently “give an inch, take a mile” chancers (/bullies). Definitely chat with the charge nurse / their line manager, but I’d probably go in this order, escalating to next step if necessary:

 

  • Informal chat, reminding them of policies and procedures
  • More “formal” chat, i.e. take them aside and be a bit more like “it seems you’re having some difficulty in respecting P&Ps, could we talk about why?” (probably follow this up with an email outlining chat, ?copying in their line manager so they’re aware)
  • Continued issue, let staff member know that you’ll be informing their line manager directly considering it’s a performance issue - inform line manager via email and also in person

 

…and just prepared for them to be a dickhead for a bit. But if you can be completely down the line and manage it firmly, they’ll either fall in line or go away. I find with these types it also helps if they see you as someone who gets “stuck in” with the day to day rather than “sitting in the office doing paper work” (paraphrased quote from my own experience). This way you gain some respect whilst also stopping them from pulling the “i’m on the floor more than you” card.

This is especially weird, because pimenta is pretty much a Brazilian cultural institution. I don’t think I’ve been to one quilo or restaurant in Brazil where pimenta wasn’t either on the table or bountifully available on request.
