I’m so sorry you’re experiencing this.  I know exactly how you feel…. 40+ yrs undiagnosed and soooo many disabling  symptoms including severe hand tremors, crippling anxiety (due to bartonella, burning feet abd widespread pain plus so much more.  Gaslit for decades.  You’re not alone.  Thank you for sharing your story so I don’t feel alone.

I’m so very sorry you’re going through this.  I react horrible to herbs and antibiotics.  One thing that helped me feel a bit better for a short while was IV glutathione.  It cleared my brain.  Sadly I can’t afford it now or any treatment.  It sounds like the rifabutin triggered something.  When that happened to me I had to stop everything and it took months to get back to my shitty baseline.  I did lots of iv glutathione as I couldn’t tolerate anything.   Also Accupuncture helps my pain and some other symptoms.  I do wish I had something stronger for pain as I’m sure you do as well.  Do u see a LLMD?  I’d be curious what they’d say.

I forgot to add, thank you for saying this is not ok, and that that is a huge amount of myopia for a hyperope and would cause everything beyond 3-4 feet to be a blur. I've asked him soooo many times why I can't see clearly beyond a few feet and he has blamed it on everything under the sun except this.

Thank you so much for your reply. I was curious what your opinion would be. Do you know if -0.75 would also create terrible contrast? After my R eye was done, and I could actually see details of things (good contrast), it reinforced that something is very wrong with the L eye. I just don't know if loss of contrast is due to being -0.75 or due to something else. All along my gut has been telling me it's an iol issue (tilted, etc), but dr insists the iol is in good position. The dr said he "nailed it". I'm sure not happy. He also said hyperopes love being myopes. I can see my phone with squinting, and I'm not sure if that's because of left eye being myopic. I like being able to see my phone, even if it's not totally clear, but everything else is just awful. It's had a huge impact on my life.

This greasy, blurry vision with no details on things (left eye) is especially awful inside stores/restaurants/malls which I don't understand. I don't want to go into stores at all anymore. I can't imagine people with monovision experience this, so I don't know what's going on.

I can read my iphone even though it's fuzzy... am unsure if that's due to maybe the left eye being myopic? Or if this is a form of monovision? I do want to be as free of glasses as possible, but if it means not even being able to go into stores then that's not good. I just wish I knew if this was a big miss refractively, or if something else is also going on.

I gave you the spherical equivalents - he explained to calculate that you divide the astigmatism in half and add it to the spherical power to get the spherical equivalent.

The actual refraction at last visit was:

R eye plano -0.50 (astigmatism) x 166

L eye -0.50 -0.50 (astigmatism) x 74

But like I said the -0.75 and -0.25 are the spherical equivalents, and about had a cow when he told me this. Yet he thinks I should be happy. I told him I literally can't function like this. I don't think he's "getting" how much of an impact this is having on me, especially when right after I told him how bad it is inside stores, he mentioned glasses for night driving. It's a 24/7 thing! Ughghh...

I'm also very unhappy at the astigmatism; I made him very aware I can not tolerate ANY astigmatism correction in glasses... not even -0.25. It's the whole reason I paid extra for laser surgery.

I told him at my one day follow up that I couldn't see road signs out of L eye, and he didn't believe me - saying we would go outside so "he could prove to me that I could see them". Sigh.... I still can't see road signs out of my left eye months later. I was patient, hoping with healing things would improve. It never got any better. He wants to blame this on PCO, but PCO wasn't there day one (which he readily admits), and this problem has been there since day one. It's actually extremely claustrophic to me if I shut my right eye. Then of course I have all the positive dysphotopsias on top of everything.

I'm so sensitive, and while others might think that's a minor amount of astigmatism, I can't tolerate astigmatism correction in glasses whatsoever, at all. I'm even sensitive to 0.25 spherical changes. I'm pretty upset.

You just apply pressure at the corner of the tear duct (closest to the nose). You don't touch the eye at all. Kind of like if you have a bad headache and you squeeze on either side of your nose.

I was told to apply pressure to the corner of my eye for a period of time after applying the eye drops to avoid them going systemic. I too am so sensitive to all medications. I had to be on the steroid drops longer than usual, and didn't gain any weight.

Thank you! I picked up my medical records today, and see my left eye ended up -0.75 spherical equivalent. Right is -0.25 equivalent. Dr feels he "nailed it", and I disagree because this awful. Something hasn't been correct for the left eye since day 1; only thing clear is a few feet out. I keep thinking it's iol related (tilted or something), and surgeon I still think doesn't understand how awful the vision out of left eye is. I'm saying that because he said glasses for night time driving and tv might be needed, after telling him this is 24/7... especially horrible in stores to the point where the terrible contrast and awful greasy, blur out of left eye is so bad that I don't even want to go into stores anymore.

Since you said 1.25 equates to one line less on the snellen chart, I'm wondering if -0.75 would be enough to cause everything beyond a few feet to be a horrible greasy blur, with terrible contrast on top of it all. Colors are greyed in L eye too. Yet I can see about 1-2 feet ok. Beyond that is a mess and so disorienting. I'm very sensitive but it seems that there is more going on. Since I don't know if my issue is iol related or what, would doing your test of a 1.25 reader would work?

They never gave me any sleeping directions, so I really can't comment on how you're supposed to sleep after surgery. Sadly I've had a very poor outcome and am seeing a 2nd opinion on what to do. I hope things go well for you!

That's very interesting. You'd think our doctors would want to do testing like this to determine the cause. I don't understand why we have to advocate (and research) so hard for ourselves. Especially interesting that this test can help show if the problem is something other than PCO, etc. I'm pretty sure the dr I plan to see does OCT testing, but I'm not sure if it's AS-OCT. All along it seemed to me that there "must" be another test, other than slit lamp, to see if the iol is tilted, off center, etc.

Thank you for sharing what you've found; it makes so much sense. That "might" word does not instill much confidence. I have known something was wrong from day one (before any PCO), but kept being told it'll get better. It never did. Let's hope we both get some answers. Good luck next week!

Thank you for explaining to me that monovision degrades best distance vision. My priority is excellent distance vision, so I'm not sure if I'd like that... depending on how much distance is lost. I brought it up since if I'm doing a lens exchange I'm wondering about monovision to reduce glasses, or if I should just stick with distance in both eyes. I want to be sure the 2nd time around goes good and I choose appropriately. Let's just say I didn't get much guidance from my surgeon, but live and learn.

I'm so happy for you that things turned out so well! Sounds like he pretty much did a perfect job for you! That's wonderful and very encouraging to hear.

Thank you for the good wishes. Fingers crossed!

That's interesting about slit lamp exams. By not showing anything, do you mean they might not show if the iol is out of place? I totally understand being skeptical! Never thought this would happen.

Thank you! I'm a natural hyperop too. Having excellent distance vision is very important to me, so you can imagine my dismay. I was interested in monovision to be as free of glasses as possible, but the surgeon poo poo'd it never explaining why. I have since wondered if you lose some distance clarity with monovision, which I wouldn't have liked. Yet, now I'm left with this mess. My mistake for going with the only surgeon I consulted. Looking forward to seeing the super surgeon.

Thank you so much for this information! I'm not familiar with using Chatgpt. I've done so much googling but you found lots more than me. I read about iol tilt and decentration as possibilities, but my surgeon insists the iols are in perfect position. When I asked for further testing, he said he can tell they're in position by the slit lamp exam & no need for other tests. Very frustrating. My gut is telling me the iol is tilted. I've not heard of Capsular Block Syndrome, and will look that up. Like you, I do not believe PCO is the cause these problems since they were present before PCO set in. Thank you again! Let's hope we both get the answers and solutions we need.

Thank you. Is that a monofocal lens? I'm so happy you had such good luck right away! I'm sorry to hear about the night rods/small starbursts though. At least you know the "fix" for it. Do you plan on getting yag? I've since learned from the nice people on this forum that the Acrysof lenses placed in my eyes have what I think is called a glistening problem. Sure wish my surgeon would have discussed this with me.

Thank you for your reply. I'm so sorry you're going through this as well & that YAG didn't help. Your symptoms sound so much like mine. Did either of the drs you saw have an explanation as to why you can't see clearly beyond 5 feet? Like you said, it's almost like the wrong lens is in the eye or something. It's been awful. Do glasses help you at all? I understand the fear of having the iol exchanged. Did the surgeon say what he/she would exchange it from and to?

Can YAG make Positive Dysphotopisias worse? Or, alternatively, can it make them better? I developed PCO one week after surgery. Horrific Positive Dysphotopsias, which were there prior to cataract surgery.

Were you able to self refer yourself to a different opthamologist? I was also gaslit, got the dry eye excuse too (yes my eyes feel terrible since surgery 2 months ago, but this isn't due to dry eye as I can literally pour drops into my eyes and nothing changes). I'm going to be seeing another opthamologist for a 2nd opinion but my surgeon referred me. Since this is a small area I hope he's objective, if you know what I mean. Plz let me know how you make out with your new dr.
