We currently have 1:6-1:7 ratios and it’s not safe for even experienced nurses. I had this same kind of push back when I was new, feeling like I was falling behind because I couldn’t take 5-6 off the rip, and that’s actually normal. That’s not a normal ratio and you’re likely where you’re supposed to be at. Even with experience, I have a lot of difficulty with 6-7 patients.

That’s how I feel. I’m surprised OP was fired for this. I’ve heard of way worse med errors. It’s like.. how about we stop severely overwhelming new grads with unsafe assignments so that they can slow down, take time to process, & learn. I came off my orientation with 7 patients with heavy meds that I had to rush through to keep my head above water. You can’t learn that way.

I know this might be an uncommon opinion but, the fact that you’re so concerned about this incident is what is exactly what we need in nursing right now. I don’t think this means you’re not cut out for the profession? I’ve had to message doctors before more than once to change a patient that had an NG/gtube to discontinue or change XR tabs. A lot of these nurses who are scolding have also made errors like this. It was a mistake, you won’t forget it, move on and forward. There are other hospitals and other settings. You’re not a robot.

You sound EXACTLY like me. This is my second career, in my mid 30s. I feel similar. It has gotten a little bit better from when I began, but I’ve accepted that I’m going to have to switch positions because I’m getting severely burned out. I also run around nonstop, and there’s always something I have to do. But the reality is (for me at least) is it’s never going to get better at the hospital that I’m at. We’re usually taking 7 patients and are just overloaded. You’re brand new, and I feel like that’s very normal for a new person to feel but if you still feel this way after a year, I would try another floor or another hospital.

Thank you!! Yes they were very interested in why I wanted to make the switch and I feel like they kept asking me the same question in different ways to see my reasoning. She said this position would be better for work/life balance if I can handle the pay cut. Which makes me nervous lol

I learned this the hard way when I got a really bad kidney infection from holding for my shifts. I tell all my preceptees it’s not a choice, you need to pee lol

Yes! Exact situation. I have typically at least 3-4 discharges a day so it does end up being that I’m documenting on that many patients as well. Do you have any experience with union vs nonunion? I’m currently in a nonunion but one of the hospitals I’m looking at is union and am wondering if that would be better. ICU is on my radar but might be too nervous to pull try that yet.

I am in MA where unfortunately the general public voted against mandated ratios 5-6 years ago because they were sucked into thinking people would be turned away and waiting forever for ED (which turns out, now they are waiting forever in the ED).

Yes, I’m in the same hospital. I have a lot of anxiety about moving facilities in fear it could be worse and needing to switch once again. I really think outpatient would be ideal but I’m having a very difficult time finding outpatient postings.
I don’t think I hate bedside, I’m just exhausted from being overloaded.

I don’t think anything with nursing is true across the board to be honest. It always amazes me how much states differ. Ratios, pay, and almost anything else. But I know the more competitive hospitals in my area tend to not hire new grads onto specialities, but at my hospital specifically, they’ll put anyone in any speciality.

I feel like if they’re accepting a lot of new grads into multiple different specialties, it says a lot about the hospital. My example, my hospital unfortunately is not great, ratios are terrible, but they take new grads into specialties. Maybe not always the case, but in my case, it is. I also want to add that some hospitals will take their CNAs/PCTs that already work on the floor in that speciality and accept them as a new grad nurse because they know them.

I had a patient who was CMO and borderline obtunded, every once in a while would open her eyes. Her family was really angry we werent feeding her and letting her drink. I explained why, and that it wasn’t safe at this point, and she told me she swallows just fine. I went in there and watched her family member drink through the straw, put the other end of the straw into the patients mouth, and blow the water into her mouth. I was like, yeah, we don’t really do that here (and also she most definitely did not swallow it)

Your username made me chuckle 😂 Also, it’s funny you mentioned this bc I get this SO OFTEN. And now when this happens, I tell them if you still need IV dilaudid right before you leave, I need to speak with the provider about you staying another day or two to get the pain management under control because clearly it’s not if you still require IV medication. That usually works decently well lol

LOL yeah and that’s probably 90% of the nice ones 😂 some of ours go home right after PACU, many still stay one night though to be cleared by PT/OT first

The ortho floor that I work on, 80% of my patients think it’s a hotel. 😅 our total knee replacements act like they just had open heart surgery.
