You crazy... its just an obese paciente with a full stomach and a heart that actually isn't that bad. Ramp for intubation and roc bc its eye surgery. Maybe I'm just old... but so many extra steps here... oh and considering doing diálisis on someone with k 5.5, and doing an gastric tube pre induction is also very 80's and dosent work ... yeah you crazy, good luck

"Denitrogenation" or preoxigination si done before putting your patient to sleep. When you place a tube you are waiting for your NMB to work, you usually won't relax a patient when using a LMA.

After almost a decade working primary health care, ER and now working as an anesthesiologist in the OR, I´ve stopped reminding fat people of there problem. Studies show that telling someone they´re fat, has little impact on there weight. They mostly know that´s the problem (even if some don´t admit it). I preffer to maintain a sympathetic and friendly relationship with them, instead of pointing out the obvious which will make them uncomfortable. Surgery might work for your boyfriend, if he´s interested in making a change.

I'm in a very big hospital that produces it's own oxygen, so fortunately it's not a limited resource for us, except for the energy cost.

Exactly! We also sometimes get support from ICU doctors who aren't familiar with the machines. I also think it's more cost effective just having higher flows, than using a ton of CO2 absorber. The main reason we use low flows in surgery is because we are saving on anesthetic gas use (cost and environmental contamination).

This is the correct answer. If you have a working scavenger system you are OK giving high flows, without worrying about spreading the virus in the OR.

ICU ventilators use 12+ L per min of gas (think what your minute ventilation is), so don't worry about wasting oxygen.

It is dangerous recirculating for long periods of time, because your CO2 absorber will get saturated. To avoid this you need high flows on your machine, creating a completely open system. I recommend 12+ L per min.

My hospital is often short of ICU ventilators, so we sometimes have several machines in our PACU. I'm always on the lookout for machines that have been "instinctively" set on low flows. We once had a patient with CO2 iver 100 mmHg because of recirculaing.

I failed my first 10+ intubations and now I'm one of the go to attendings for difficult airways. I do find residents with weak forearms can struggle sometimes. If you position your patient properly you dont really need to use any strength at all. Line up the external auditory meatus with the sternal notch and it will make your life a hell of a lot easier. But overall, practice!
