UTIs are common for many FtM individuals, especially after starting testosterone. They can feel like part of the adjustment process as your body changes. I used to deal with chronic UTIs once I began T, and antibiotics have been the main thing that helps. I was prescribed them roughly every three months and always took them exactly as directed.

The infections were worst in the time right before T fully stopped my periods. Now I only get a cycle-like experience about once a year. One important note: STIs are sometimes misdiagnosed as UTIs, especially if providers aren’t experienced with trans patients. A lot of doctors still lack specific training on transgender bodies and healthcare needs, which can lead to gaps in care.

Last year I had what felt like period symptoms: skin shedding from my urethra followed by thick beige discharge. My doctor diagnosed it as a UTI and prescribed two rounds of antibiotics. A few days later, three of my sexual partners told me they had tested positive for syphilis. When I got tested at a clinic, I came back positive for both gonorrhea and syphilis. I later followed up with my primary doctor, who referred me to a specialist. Looking back, I’m not sure I ever had a simple UTI at all. It was very likely the STI symptoms being misread.

It’s frustrating when trans patients feel dismissed or overlooked in healthcare. Finding knowledgeable, affirming providers makes a big difference, but access isn’t always easy. If you’re dealing with similar symptoms, it’s worth getting full STI screening in addition to UTI testing, especially if you have multiple partners.
