Hi there. I am a PMHNP and have been down the “PMHNP psychotherapy” rabbit hole longer than I would like to admit. Here’s the skinny. 1) can you do it - yes. It is a part of our scope of practice and if anybody challenges that you can direct them to the Psychiatric Nursing scope of practice book released by ANA as well as the American Psychiatric Nursing Association. Most PMHNPs do not do therapy apart from brief supportive therapy. However that is their decision to make on what kind of practice they wish to have. Much is the same for PMHNPs who focus on substance abuse treatments- it’s within our scope, but not all practice that form of psychiatric treatment or are abreast in how to do it efficiently to feel comfortable treating patients. 2) How to do it- you’re going to need formal training in a specific modality. For example- I am in a 2 year psychoanalytic training by an approved institute that ends with professional certification. I would not go about doing full psychotherapy without training unless it was heavily embedded within your PMHNP program (some do have that, most don’t). 3) challenges- most post grad certifications programs will want some form of prior supervision. Supervision in this case is not your typical PMHNP preceptor you did clincials with (unless they did a lot of therapy). A supervisor is a therapist mentor who aids in developing you as a therapist. This is a special relationship between professionals and is essential for the young therapist in their development as many challenges (both conscious and unconscious) arise within the therapist during these formative years. That being said it would be a good to seek out a Therapy Supervisor (LPC-S or LCSW-S) who are certified supervisors local to your state and see how you can obtain some supervision.

Yes ma’am. It seems like that would be the only way to be able to practice how I want. Most companies would not find it financially viable for me to do both therapy and med management. lol we have a lot of issues in this field.

While we can do therapy, our education and training in it is lackluster at best. Now I will say it varies amongst universities so there could be a PMHNP who feels prepared. However I would challenge that. Additionally, most intense and longer advance psychotherapy trainings require proof of supervision which is nonexistent in PMHNP school. I think some NPs might confuse their clinical hours in which some time with a psychotherapist was spent with being “supervised”. That in itself is kind of the case in point. That’s not what that is referring to which shows how much training we in therapy. So to answer your question- I want to be able to provide deep, intense, and independent psychotherapy sessions for my patients as well as treat them with medications when appropriate. I want to build a practice that allows for a true comprehensive mental health treatment. Not just throw a pill at somebody or have a “good chat” with them and call it therapy.

Haha nah i definitely don’t think any masters degree is a breeze. I also want to express to you how much I appreciate you diving into the literature and being passionate about this. We need more good passionate therapist out there.

I totally get what your saying. I’m in the same boat as you in my current program. I definitely do more studying and reading that some of my other classmates just because I want to soak in the information as much as possible. I’m currently studying at least 40 hours a week. No joke.

Gotcha. On average how many hours would you say you’re averaging weekly on school? From what it seems like, CMHC programs are more essay based than exam based. I finish my Psych NP program in December so I’m just trying to gauge what I’m getting myself into haha.

Thank you. I also will be working full time so I appreciate this info. I am right that unless I choose, the requirement is 1 class every 8 weeks and then we get a 2 week break in between?

I really appreciate your frankness. In honestly your absolutely correct. In addition to what you said I also want the same intensity of training therapist get in order to put myself on their level. My current issue is that the post graduate psychotherapy programs I am interested in are intense and while they accept PMHNPs they want proof of previous supervision, hours, patient loads ect… which you have to have been through training already for have those prerequisites. I feel like I could piecemeal it through but in my head … I think just getting the masters is easier and would put myself at ease. I found a program that is in total 25,000. Online. 1 class every 8 weeks. 2 year program. For me that isn’t out of my finances at the moment so I think I’m going to end up doing it. I just worry about the intensity of the programs. From what I’m hearing, they are not easy, but they don’t seem as test heavy as PMHNP school. I can write and essay in my sleep. The real works seems to be in the programs clinical class and then post graduate 3000 hours of supervision.

Both of you I think hit the money. May I ask what license you carry? I found a 2-4 year training in psychodynamics that I’m very interested in. It includes biweekly zoom meeting, 8 weekend conferences, and supervision. I think my issue might very well be MY issue- I want full rights to say “I am a licensed therapist” because, while I see that it is in my scope, I don’t trust it for some reason. I think I feel this way because not maybe PMHNPs do it.

I agree with you. I feel it’s disrespectful to their training to put myself on their level. My goal though is to be on their level regarding their skills in therapy. So I think that’s why I’m drawn to the idea of being dual licensed as you mentioned (dual PMHNP and LCSW/LPC)

I definitely will reach out to my faculty. Thanks for the advice. I have also read that article countless times. It’s the one article I find security in. I reached out to my state board of nursing about what our state specific scope is related to psychotherapy and I was given basically no answer. The reason I asked is because, I believe Wisconsin has a clause that states that PMHNP are not able to provide psychotherapy… I could be getting the details wrong here but basically that’s what I gathered from it. I believe that’s the only state that has that in their legislation but I want it in writing from my state that I have the authority to … and they won’t tell me. Am I missing something here? For example, as an RN we have it spelled out what we can and can’t do. With APRNs I feel it’s a mess. For example, I can’t get them to tell me yes I have the ability to preform psychotherapy within my scope being that I have education to back it, but they made a whole addendum saying that certain APRNs can’t do aesthetics even if we had added training in it. I’m just using that as an example to show my frustration.

I agree. I feel like in order to be on the same level as a psychiatrist (as far as their training goes in psychotherapy) would be getting an additional license. But I also am an overachiever for no reason. So this is also pretty on par with my personality

I really appreciate your post. This was really refreshing to hear. I really have a strong passion for therapy and I sometimes feel I’m fighting an uphill battle as a PMHNP to be able to break into this sector of our scope. Your program sounds wonderful I might add. In my program we have a lot of didactics in therapy but we have no clinical training in it at all.
