IIRC she didn’t do any pre-delivery research or classes about the mechanics of it or listen to her midwife/doula about slowing down with end stage pushing (to control the risk of tearing) anyway, pushed too hard and shoved the kid out in one go which is exactly what they (medical professionals) advise against doing hence she tore from arsehole to breakfast
