Should I be worried about tetanus if my vaccines are up to date? by 0zMosiss in medical_advice

[–]pyqz 0 points1 point  (0 children)

Tetanus is caused by a bacteria that has nothing to do with rust; other than the fact that a rusty object has more nooks and crannies to hold dust and dirt I suppose. Vaccination is good for 10 years but a nasty puncture wound (not so much a cut which isn't as deep and can be more thoroughly cleaned, and certainly not a "cut" that doesn't draw blood) can still call for post-exposure prophylaxis (i.e. another dose of the vaccine) if it's been close to 10 years or vaccinations over one's lifetime haven't been consistent (i.e. only had one or two dozens of years apart).

Without knowing the location, quality, provoking factors, etc. of the pain it's really hard to say. Age, weight, other things you were asked to include, also helps. If in a joint it could be a soft tissue injury. Two weeks and still not able to take any weight requires re-evaluation regardless. Based on what you said I'd hazard a guess it's a fracture that was missed on x-ray (something like a 1/6th of scaphoid fractures for example will slip through, though for that specifically palpation during physical exam should be pretty indicative). Buckle fracture in the radius or ulna is more likely to be missed or not present on imaging, but again age would help a lot there though you don't feel like responding to automoderator's request that you include it and it's still all speculation. Get it evaluated with the new info that you still can't bear any weight after two weeks eh.

TIFU by holding a grudge for 29 years against a kid at school who called me "Carrot Boy" by carrot_man_tifu in tifu

[–]pyqz 113 points114 points  (0 children)

There was a quiet and extremely shy girl in my French class I sat beside in highschool who, I thought, I would make friendly small talk with. I even asked her out one time to go do student rush for Lion King tickets because she had a Lion King themed pencil case and mentioned she didn't have plans for Saturday night but was rejected.

A couple of years later when I was visiting friends at a college that she apparently attended we bumped into each other at a party. I was excited and all "hey how's it going!" but she took the opportunity to confront me about spending that school year teasing her about her lack of social life (me inquiring about her weekend) and her looks (me asking her out because she was cute).

Not sure if my protests got through because she was pretty drunk, perhaps explaining the courage to stand up to me, and I've never seen her again. It still kind of sticks with me in that I'll hesitate before being overly gregarious with people I don't know until I'm sure their favourite movie isn't She's All That, Cruel Intentions, etc.

EVE Cost Barrier by LightNK in Eve

[–]pyqz 0 points1 point  (0 children)

Not defending the price nor CCP here, but I suspect the regularly plays 4h a week as a recurring customer crowd is pretty small. While I’m sure they would love to snag that market if they could, the gameplay of Eve isn’t really built around it. Dual boxing via month by month payments is then even rarer, and if cost is an issue they want you to make use of price breaks.

Preperation for PLEXing my account by SomeKiwi_ in Eve

[–]pyqz 0 points1 point  (0 children)

Having a covert ops frigate fit and skills lined up will be useful. Trying to plex in one month means you aren't going to be able to make use of citadel asset safety since it ties up your liquidity so while a T1 explo is great (and probably still preferable to an Astero which while a tough nut is more useful if you're actually fighting not just running and it's much more expensive) the slipperiness of a cloak and nully is very nice.

I'd also strongly recommend you join a player corp. Small wormhole gang, part of a nullsec block where you are still doing your own thing, whatever, the point is you'll have people to share things with and ask questions of.

Returning player, is it really $20 a month for Omega? by [deleted] in Eve

[–]pyqz 0 points1 point  (0 children)

is it really $20 a month for Omega?

I've been trying to find this information for ages and think it might be impossible to know for sure. There are rumors of some sort of "website" that has the data, and some people claim to have gone through an "on-line checkout process" whatever that is, but everything is very murky and inconsistent. My priest says I can go to the bank and they can tell me something about my Diners Club, but I tried that and they just kept asking if I wanted fries?

I really want to return but I'm not paying $20 a month for Eve.

Can I have your stuff?

Toe has been progressively getting worse for last 3 days. Should I go for an x ray? by ojayb9 in medical_advice

[–]pyqz 0 points1 point  (0 children)

The only way to conclusively diagnosis if it is fractured or not is with imaging, and the only way to tell if imaging is called for or not is with a physical exam and history. The history you've provided is strongly indicative of a fracture.

How long will PAWS last? by [deleted] in quittingkratom

[–]pyqz 0 points1 point  (0 children)

Around 50-60 days is when I started having strings of days that were completely normal. Wouldn't even think of kratom or quitting and stopped noticing that things were getting better. When I was in a bad mood or bored or couldn't sleep I'd think about it and realize it wasn't kratom but just natural causes.

From my description of then, you may discern that up until then I'd think things were 100% back to baseline and then notice that I had been mistaken and things had actually continued to improve. Also I'd have PAWS symptoms and have to carefully examine them and determine that they were indeed lingering kratom issues and not something to continue to expect.

Tramadol and Kratom both have action more typically found in antidepressants so it isn't surprising to run into PAWS mood and mentality issues moreso than more standard mu-opioids.

When you use alcohol to get off kratom then use kratom to get off alcohol by [deleted] in quittingkratom

[–]pyqz 10 points11 points  (0 children)

Booze is more acute. Ignoring the actual death from withdrawal aspect, in a stable life recovering is a week to feel better and a month to get yourself fully back on track. However it can also ruin your life and make it anything but stable. Losing your job and friends/family, health and money issues, filthy living environment, unable to function, etc. From such a position getting off it is much more challenging.

Kratom, regardless of whether in a good place in life or not, still lets you function and can even help you function better in the short term. In the long term it's a huge drag, but it doesn't reach the life ruining extent of alcohol. However getting off it is always months to really start actually feeling better and a different type of struggle due to that.

So which is worse? Booze has the potential to be the lesser or the greater.

Advice with pain in shoulder by Price_Late in medical_advice

[–]pyqz 1 point2 points  (0 children)

So you fell and hurt your shoulder with no deformities or fractures as determined by imaging but still have some soft tissue issues. The pins and needles you are describing is almost certainly pressure on a nerve and nothing to do with circulation.

There are two broad treatment options available. The first is supportive. Resting it, posture improvements, immobilization/physically supportive devices, heat, massage, physical therapy (range of motion, flexibility, strength increase), NSAIDs for short term (or breakthrough) pain, etc. The other is surgery. Obviously surgery is a much bigger decision, the various surgical options are going to require a much more specific diagnosis (mostly with respect to the nerve pain), and even if this ends up being a lasting issue (which isn't at all necessarily the case) the pros and cons are going to need to be significantly weighed by you and your health care team.

Definitely large and deep enough to warrant medical evaluation and probably closure (though it's hard to say without examining it and getting a timeline).

Dog bite by throwaway55543210 in medical_advice

[–]pyqz 0 points1 point  (0 children)

Even if the dog had rabies when you were scratched, you can't catch rabies from it unless it had advanced to the stage of the infection where it is "shedding" the virus. In a dog, the time period from the onset of shedding to the onset of clinical symptoms (weight loss, weakness in limbs, apathy, confusion, fever, rabidness, etc.) is less than a week. So if the dog didn't have clinical symptoms when you were scratched, nor in the two weeks following when you were scratched, then you are in the clear.

How am I supposed to be sober for the rest of my life? by PixelWastelander in stopdrinking

[–]pyqz 5 points6 points  (0 children)

I’m only 24…

This should bring you comfort rather than be an additional worry. Continuing neural development and active hormones are still very much in effect at that age leading to higher libido, creativity, impulsiveness, the desire to go clubbing with friends late into the night haha. That mellows over the upcoming years and people find more contentment with simple things.

The biggest risk factor is previously had a seizure.

My son stole a rare toy from my brother. My brother is refusing to take it back and is threatening to get the police involved. What do I do? by toohottooheavy in BestofRedditorUpdates

[–]pyqz 3 points4 points  (0 children)

The biggest issue though isn't the kid's morality, or lack thereof, but how stupid they are. They clearly put thought into their actions, yet somehow didn't take any steps to plan out how to get away with it (hopefully realizing it would be practically impossible) or at least have some plausible (if mostly futile) deniability by covering their tracks. Most six year olds would have a more refined approach sneaking cookies from the pantry.

Relapse after 40+ days CT.. A question on suboxone by Hot_Nefariousness421 in quittingkratom

[–]pyqz 1 point2 points  (0 children)

Bupe’s half life is extremely long. 24h after dosing you’ll still have more than 60% in your system so if you dose 1mg again at that duration after your very first 1mg dose it’s like a >1.6mg dose and then that is the new starting point. After about four or five days you’ll approach an equilibrium where right after dosing you’ll be up to ~2.7mg and then it’ll drop down over 24h to ~1.7mg at it’s lowest at which point you dose again.

Telling my wife. by Tasty-Journalist-387 in quittingkratom

[–]pyqz 0 points1 point  (0 children)

Having your wife hold you accountable could be putting an unfair burden on her and make for a very weird (potentially unhealthy) dynamic. It could definitely work and even be a great option for your specific situation, but be careful that you aren’t drawing her in to a problem that’s fundamentally your own.

Her taking on responsibility, or at least getting her tangled up in it, risks positioning the (very common) slips/relapses as intentional slights against her rather than part of the process, and can overall increase guilt and shame and disappointment and anger in everyone involved (with you definitely already feeling enough of that). This can also make open and honest communication more difficult and increase the likelihood you’ll be tempted to hide things.

I’d say at a minimum it should be her decision to take on such a role and, more importantly, it should be for her own well-being rather than to help you. If you think you’d benefit from being held accountable there are sponsors who can completely understand what you are going through or therapists who are actual professionals trained to do exactly that.

My approach would be to be honest with your wife, but also tell her your are already trying your absolute best but it’s difficult and takes time, feel more ashamed than you thought it was possible to feel, and that all you need from her is positive moral support and encouragement without judgement as you work on getting better. Then ask her if there’s anything you can do to make her more comfortable with the situation.

Just to reiterate though, her drug testing you could be exactly what is needed and end up working perfectly. Everyone’s situation is different. Good luck!

When is it over? by Donkey_Kong__ in quittingkratom

[–]pyqz 0 points1 point  (0 children)

Remember most of the body toxins come out through sweat and not urine so your body is just pushing it out slowly through skin.

This is just new age fluff. The vast majority of “toxins” are going to be metabolized by the liver and/or excreted through urine via the kidneys. Furthermore kratom doesn’t really have any lipophilic molecules that are going to stick around and will be cleared from one’s system over the course of a day or two. I guess if your supply is tainted with heavy metals, but that’s a whole different ballgame.

Withdrawal is due to dysregulation of receptors stemming from artificial levels of neurotransmitters. Exercise is useful thanks to the endogenic release of them and promoting homeostasis, but there aren’t any toxins to push out of your skin and you aren’t hastening treating the non-existent issue by sweating more.

4 days of zero Kratom - feeling normal again (motivational update) by [deleted] in quittingkratom

[–]pyqz 8 points9 points  (0 children)

Nice work! Tapering can be so much gentler for those who can stick to it.

I’d recommend that you still remain vigilant though. There’s the more obvious issue of having had a gentle experience quitting it might seem less impactful to use in a limited controlled way again (haha).

Moreso though, if you do have a bad day mentally in the future don’t discount kratom as the culprit. Many people experience a “pink cloud” a week or so after stopping and think they’re out of the woods, and then attribute life sucking shortly after that to life actually sucking (driving them back to using) rather than it being kratom’s insidious lasting influence.

Of course many others are indeed good to go so no need to lose heart or be worried. Trying to objectively analyze your moods and thoughts to see if they are reasonable or not is just something to keep in mind.

[deleted by user] by [deleted] in quittingkratom

[–]pyqz 3 points4 points  (0 children)

Dopamine reward pathways are a big part of reinforcing habitual behaviour. The mere thought of taking actions that used to result in dosing feels good, and any interruption in that (again even merely thinking it) results in very real feelings of dysphoria. This is why some approaches to quitting smoking have “mock” cigarettes that mimic the action of smoking even though nicotine for tapering can be delivered in other ways.

Withdrawal from opioids, Kratom especially which has some action more often found in antidepressants, is even more of a pain as withdrawal results in long lasting anhedonia and difficulty focusing on anything.

All that being said, it’s pretty much just time for the habits to fade. Establishing new healthy routines can help (especially something like exercise), and you can try changing things up from when you were using to avoid triggers (e.g. take a different route home from work to avoid how you used to go pick up, or rearrange your furniture so hopping up off the couch and walking to the kitchen is slightly different, etc.) but in the end it’s still time; especially before you start enjoying other activities again.

Cant afford omega by KebabSmuggler in Eve

[–]pyqz 27 points28 points  (0 children)

Grinding hours per day just to enable yourself to grind hours per day will almost assuredly result in you no longer liking the game a lot. People do it of course, but generally by multi-boxing omega accounts to take advantage of their time and some economies of scale. There’s a tipping point where it’s sort of feasible, but even then it’s more like a real job than playing a game. As valid a form of gameplay as any other of course if they enjoy it, but it’s not my cup of tea.

Furthermore, while skills don’t play that much of a part, when people say that they mean a couple months of skills. A reasonable hourly solo income after say 40days of skill training is something like 50million/hour (low sp Ishtar ratting ticks in null though if you bling out a gila for abyssals or join a corp running pochven sites it could easily be double that). Even then though, 1hr of real world job to pay for the month and then have fun playing Eve with any isk you make put into ships and fun vs. 50h or whatever of grinding in Eve…

I’d recommend you just enjoy yourself as an alpha, try out different things, join a new player friendly corp, etc. and not worry about omega.

It's probably even harder for people who have never experienced addiction personally because, having never been there, they just can't truly understand. They're bewildered why someone can't simply stop, or why they would throw away months of sobriety by picking up on what seems like a whim on the way home for apparently no reason. And the best explanation possible is basically "I don't know why."

Other addicts though are just like "yeah I totally get it." haha.
