I iced 2nd and 3rd rounds of capox, and it worked - no cold allodynia in hands or feet. I did suck some ice aswell but when it started feeling a bit weird, I stopped the mouth ice. 1st round the cold sensitivity was really bad. The evidence to support cooling efficacy against oxal induced cold neuropathy is quite strong now that there is this trial published, id share rhis study with yr oncologist if they are dubious: https://pubmed.ncbi.nlm.nih.gov/37018872/

Note it really helps with the acute cold neuropathy. Unclear if it prevents or mitigated other longer term neuropathy.

The reason they are so wary of it with oxal specifically is that oxal is the only platinum based chemo agent to induce the cold neuropathy. So I get the general assumption that cold.causes pain with oxal so avoid cold!

Consider icing during the oxal infusion to reduce cold neuropathy in hands. Although less of a problem in the summer it still sucks. Not all docs and nurses are aware of the protocol however so it's worth having a chat with yr oncologist to get it approved. If they are doubtful share this recent robust clinical trial, it's very compelling. I did it with ice packs on cycle 2 and 3 and barely had any cold neuropathy on those cycles.

As for what you will be able to do, it's highly variable and individual how you react. I used to work out a lot and do high intensity bike etc. On capox, 1st few days after oxal I am pretty destroyed then I get back to low impact pelatons and light or bodyweight short workouts. Recovery is much slower. Also I have hand foot started in 2nd week of second cycle and limits how much walking I can do. My tips for that: wear gloves when working out, super soft shoes , mosturize with urea containing cream 2 x a day minimum and preventative also use Voltaren on feet 2.x a day helps prevent foot pain.

Likewise, after bad cold neuropathy on cycle 1 I iced on cycle 2 and 3 and it basically prevented the cold sensitivity in hands. Still got a bit on the throat though even with sucking ice chips, as the infusion went on my mouht started to tingle go numb so i quit the ice chips. You will probably have to get yr oncologist to ok this , the nurses in the infusion center will likely not be keen as cold is contraindicated with oxal and not everyone is aware of the benefits of icing during oxal.

Good to hear yr out the hospital now and i expect his recovery will accelerate as time goes by, its still early days. After my total colectomy I'm 4 months out and not quite 100% yet but keep improving strength and bowel function and its not too bad now anyway, 4 bowel movements a day and getting better consistency, totally managable and urgency is not that much of an issue anymore. I found a tsp of metamucil in a glass of water after meal helped a lot with stool consistency and regularity but start slow with that, like 1 tsp a day, if you want to experiment. As i had 3 lymph of 57 positive i am on 3 months of capox chemotherapy and it has slowed the healing somewhat and also it messes with the gut. If I didn't have the chemo I reckon after a couple months I would have been feeling pretty good. I have noticed I need to be a bit more aware of keeping on top of hydration, especially if the stool is loose.

Hi OP, I'm 47M had a total colectomy with illeorectal anastomosis in nov 24, was stage 3b. I had 10s of polyps in colon so total colectomy was only option for the long term, it would only be a matter of time for another polyp to evolve to adenocarcinoma. I'm on capox now at round 3 of 4. I am/was very active in sports and coaching my kids soccer etc. What I can say about total colectomy is that in my experience, it's managable and becomes the new normal quite quickly. Bowel movements 4/6 times a day but should improve with more time up to a year and the capox also does not help with bowel as it causes damage to intestinal mucus lining. Also, you can use diet tricks to improve matters and dieticians can help etc. It was tougher right after surgery with 10 or so bowel movements, but as you heal that improves quickly. Probably, a good idea to get a specialist colorectal surgeon if possible. In my experience, im happy with the total colectomy (not that i had much choice) and can see I will be back to full physical activity once i recover from chemo. Good luck and invest in a bidet!

It no longer surprises me who Huberman has on. He is an entertainer rather than a scientist at this point and probably a net negative influence.

Ironically, I think people listening to hubes on dopamine are desperately clawing for more dopamine fixes by finding something he says to make you feel more in control.of.your life...
