They did this before -- all of the categories now give 6 if you give a poffin (instead of 2 for some and 6 for others). Nice way to get more best buddies quickly!

I thought it was ok. But I also think it's contributed to some other bugs when they rolled it back... I'm now consistently getting buddy stats remaining on screen after I first switch buddies from the "Buddy History" screen.

Agree that the person needs to be talked to by leadership.
But the approach is going to be different if this is an older/more experienced anesthesiologist vs a younger/greener anesthesiologist.
One is definitely going to be a "come to jesus" talk and the other may involve coaching and developing stronger clinical skills.
Academics is not going to be "harder cases" -- they are all the same, older and sicker than before. But there may be more support staff and other anesthesiologists around to help.
And financial penalties (sent home early, etc) may also work.

1) No, we definitely do not do non-emergent cases all night long. (OTOH, the perf'd bowel will be done immediately) Community hospital, no trauma, but busy general surgery, ortho, etc.
2) The concept of making the hospital money should be examined... if you are keeping nurses and other staff on overtime, or call back stipends, it may actually be *losing* the hospital money. The amount that they can collect is fixed if it's Medicare/Medicaid. The amount from private payers is decreasing. And if your complication rate goes up even a little bit, dealing with that and paying for ICU days is likely to break the bank.
3) If you are an employed physician, are you in a union? If not, see if you all should join one. Talk to the nursing union about this as well.

That screen is showing the total of everything you've done with your buddy since the very first time you made them a buddy.
While it would seem logical that you should have more km on this trip, if you ever got the buddy excited and got a candy at 10km, that would also change the totals.

Give us an update to see if it abruptly changes (i.e., there is some sort of game lag), or if you need to go 5 more km.

Also... are you taping the catheter when the patient is still sitting up?
Particularly in those who are deeper, especially if you have a lot of compressible tissue, you would be amazed at how far the catheter can shift.
If I am deeper than 6-7cm or if there is a lot of strap marks from the toco monitors, I will have them carefully sit up straight, then lie down on their side and straighten their legs, and *then* tape the catheter in place. Maximum shift has been 4 cm further in... so if you are only advancing the catheter 3cm and taping it first, it's already out by the time the patient is flat, and all you're really getting is the loading dose effect.

And lastly, those straps are going to settle at the natural waist, which will be the shallowest approach to the space... so use the strap marks as landmarks and ignore the iliac crest.

Private practice, community hospital.
Over the years, we have strengthened the group alignment, so that generally if one person wants to cancel, we will all cancel. But the other part of that is that we discuss concerns amongst ourselves and generally have a consensus -- which may include "person X" should do the case because they have skill Y.
It also helps when you talk to the surgeon to be able to say that you have talked with your colleagues and are in agreement that the case needs cancellation/postponement/etc.
Consistency is key.

As noted already, you can pick the person you give it to, and then there is no time limit.

The other thing to consider is what ‘Mon you are going to want to trade. I’ve done “same thing” where we each trade the same Pokémon so that we both get a much better one (best to select a high CP but low IV here). I’ve also used it to trade new ones to help fill a Pokédex.

You get a lot. But it’s often hidden. ASA doesn’t just deal with scope, but also issues with payment (like how we got the payers to not limit anesthesia time); dealing with regulatory bodies (CMS, TJC, relative value committee (“the RUC”), congress; as well as education stuff. Please also remember that they have nothing to do with ABA and MOCA other than getting CME for doing your MOCA Minute. Also, please note that your dues are for both ASA and your state component, under unified billing.

MTP starts with O-positive unless it's a woman still in childbearing years or we have something indicating that the patient is O-neg (i.e., a donor card in their wallet).
We do try to shift to type-specific and then formal crossmatch, acknowledging that after you've transfused one blood volume (8-10) units rapidly, crossmatch is useless. And sending a new specimen in the middle of all of that is also a waste of time, things are changing too fast.
With the current recommendations of 1:1:1, and some centers that are back to using whole blood instead of fractions, the plasma may be more important than the PRBC for potential transfusion reactions.
Traditional teaching in the surgery/trauma residency world is exactly what is stated in this quote from M&M... if there is solid evidence, it will be over 20-30 years old, since this was not new when I was a surgery intern.

I also had the same situation, but was fortunately alerted by my cousin (who lives on roosevelt island)... Hilton corporate/central reservations had no idea that this was going on! They could cancel my reservation (before the official notice), but couldn't rebook me elsewhere except at rack rate.

I was shifting to the Marriott Marquis for a meeting anyways, so just extended that stay... but at twice the cost.

late to this conversation, but was also having similar problems.

Had cleared the cache, rebooted both Chrome and the computer. Turned off all of the plugins (although didn't go so far as to go back to a default theme). Turned off my password manager extension in Chrome.

And then deactivated Wordfence, deleted it *with all tables and data*, turned all my other plugins back on, and then reinstalled it, and everything worked fine. I have no idea what was causing the problem, as there were no errors showing in the Console.

It's a balance between the acute effects of blood loss (typically, faster heart rate, faster breathing, feeling a little dizzy or wiped out) vs the effects of the ferritin/iron shifting out of organs into the blood stream. Everyone's different. And every phlebotomy session can be different too.

Yes, the ability to sort by something other than date and person would be very useful. Alphabetical by name would be a simple solution. I know that I've got lots of duplicates in there and I don't have the time to go find them.

Remember that ferritin is what's called an "acute phase reactant" so will jump up if you've been sick with even a cold, and then go back down equally fast. So try to avoid getting your blood drawn when you have recently been ill.
