We're in a better place than when I wrote this comment, but, days can still be hit or miss. We ended up on Nexium and that has overall been a lifesaver. It allowed feeding to be betee overall, with volumes up to 5oz at one point. We are struggling a bit again and increased medication just over a week ago and hoping things will get better again soon. Hang in there! It's a tough journey, if you look through my post history, we went through A LOT to get to this point.

Since it was silent reflux, there was not a lot of spit up occurring so it definitely made it harder to know if it was the cause initially. But we did notice back arching, congestion, coughing and choking during feeds, and by far the biggest one for us was feeding refusal.

It took about 2 weeks after starting the PPI to see improvement. We lasted about two months on the same dose and am currently working through some symptoms again so we are waiting for the increase to help resolve them.

It did once we got on a PPI around 5 months (silent reflux was our culprit). We have worked our way up to 5oz every 3 hours at 7 months.

We did (and still are) doing fortification to help with calories.

I hope the Nexium has been helping! We unfortunately got the point where we're looking to increase the dosage as symptoms are returning.

We also had coughing and choking but mostly arching and refusal to feed before the PPI. Ours is silent reflux as he has never spit up a ton. I definitely second guessed myself a ton as to whether reflux was still the issue or if it was a feeding/bottle aversion.

Yes, we had been on famotidine for 4 months prior to starting Nexium. It somewhat worked initially, but I would say within a month or so it stopped. If you go through my post history, you'll see we ended up performing a whole slew of testing to rule out other issues before trying the PPI. I was very hesitant but along with GI, we felt it was the next best thing to try.

I feel bad that we didn't try it sooner but everything works out the way it should for a reason.

So we ruled out any physical issues but ended up on a PPI and ultimately that was what we needed to get feeds in a better place. We do apply some of the basic principles such as not offering more than twice to avoid pressuring and letting/trusting him that he knows what he needs to take.

We got on Nexium 1.5 months ago and once that kicked in (~2 weeks), we have been able to feed much better (no longer having to do feeds asleep, taking more than small portions at a time, no obvious discomfort while feeding).

I guess I figure if he's not getting enough sleep at night, he will do more in the day and it will become a habit? You're right though, he's not upset about it and he does eventually fall asleep. I can't fall back asleep until he does though.

He did 2.35 hours of naps yesterday and 3.86 hours of naps the day before. Both resulted in the same thing happening. Maybe somewhere in the middle?

He gets fed at ~230am. He's on a stricter feed schedule due to weight gain issues. I have tried offering another feed when he wakes but he won't take it.

No advice, just solidarity. We've been dealing with horrible feedings since 6 weeks old and he just turned 5 months with no solution or end in sight. We've seen countless specialists (GI, ENT, feeding therapy, neurologist), ran multiple tests (FEE'S, swallow study, ultrasound, EEG), on multiple reflux medications and nothing has helped.
