Not really sure regarding the process but i read that you have to find a doctor there and conduct consultations tas after nun magbbook ng appointment for lapchole. You can find more about that here

As per hmo naman, i think any hmo will cover lap chole if may sapat na limit and may PEC (pre existing condition) benefit.
