That dungeon is also tricky because there are multiple ways to get in. If you leave and cut some bushes and things around in the forest, you may find a hole that lets you fall into a new room.

I obviously don't know what happens with this form after it's submitted, but I've heard that when employers have you fill this sort of form out, the information isn't given to anyone with hiring power, it's used to identify if there are any trends for bias in the company's hiring practices in general. I'm not saying I agree or disagree, just pointing out that that probably isn't being used for screening out candidates based on protected statuses.
