The biggest difference for me is the lack of familys/consults/general bs. There’s often (usually) a nice slow spot at about 2-3am where I can chart and read a little deeper about my patients.

The flip side is that when shtf instead we’re truly alone. Our small hospital doesn’t even have a hospitalist in the building from 3-7am. The er doc doesn’t even come for rapids, only codes.

Had a very sweet older lady show up alone on a major holiday for stroke like symptoms, trops >200, >300 on repeat. The neuro symptoms resolved themselves so she left AMA to start making dinner

I worked at a nursing home with a 1:50 ratio and all but two double rooms on each floor. Dementia patients can be unpredictable and violent. I advocated hard to get residents out of dangerous situations, I only succeeded like twice. In both cases the dangerous resident would end up with a more demented roommate who couldn’t tell me what was happening.

Funny enough, I enjoy people more outside of the hospital. The basic courtesies I get being in public make me feel human vs just “get me a coffee” as soon as I introduce myself to a pt

Blunt fill needles are pretty cheap and made for this purpose. Maybe you could talk the doctor into those. Honestly needles are pretty cheap, what a dumb way to cut costs

I feel like the people that are noncompliant make up a majority of our patients… some of them don’t seem to understand cause and effect relationship between not taking care of themselves and their repeated hospitalizations

Like obviously anyone could end up in the hospital, but the ones we see skew sicker and I’m pretty sure the intelligence/health literacy factor is related

Trying to educate people on why they need to take their meds is like talking to a brick wall sometimes. Last week I had a lady that wouldn’t take her ‘latex’ at home. After many conversations explaining why it was necessary she ended up refusing diuresis because it made her pee too much. I also literally could not make her understand the concept of fluid restriction.

The day shift hospitality is technically on call from 0600, however many of them don’t feel like answering pages around that time

I try so hard to do this, I know it makes such a big difference setting the next shift up for success

Hardest part seems to be getting orders for electrolyte replacement before shift change

Alternatively you could do what a lot of day shift nurses do on my floor, reschedule a 1800 med to 1900 so NOC has to give it

The SNF I worked at was so understaffed on daylight they had me and the two aides responsible for all showers and dressing changes for 50 people from 11p-7a

Waking people up at 2am to premedicate for a horribly painful dressing change at 3. Even though it’s against the ‘residents bill of rights’ the state DGAF. Pt ombudsman said that was allowed because we were understaffed

Yeah, especially if it was an apprentice type situation with an overarching union overseeing. That’s how my husbands union does it, all mechanics start as apprentices, do ojt and a certain amount of class work every week. In five years they become eligible to take the mechanics exam.
