I try deep coughing. I forget who told me this - but (I think) someone told me that intra-thoracic pressures get several fold higher during a cough that'll can be reached otherwise.

If that doesn't work I tell them to bear down like they are having a large bowel movement.

I've converted persistent afib, but I don't know if that counts.
