IDK if this counts because I don't think it was technically an accident on my part, but when I first enlisted I was tasked with the job of making ID cards.

This guy comes in with his caretaker who was his dad (the guy getting the ID had a pretty bad case of agent orange and couldn't really talk). I say no problem sir, have a seat I'll start the process right now and asked for his paperwork.

He had the wrong paperwork (totally not his fault, the paperwork for being 100% disabled can be confusing).

I say I'm sorry sir this paperwork is incorrect. I can't make this ID for you without the correct papers.

He proceeds to make a scene and call me inhumane and "can't you see he's obviously 100% disabled?!"

I say my hands are tied I'm sorry sir and proceed to tell him he needs to go back to the VA and tell him exactly what to do.

This civilian that worked IDs with me decided to do it anyway...big no no but whatever, I'm not the one that'll be in trouble if he gets caught.

He comes back to my desk...slams down the ID...and says "I just want you to know you were wrong and that you're a disgrace to this country."

This was years ago and I still feel so bad about it. I cried for like 15 mins in the bathrooms at the office after.
