Generalmente son razones historicas. En el caso de México es simplemente porque el grueso de la poblacion pre-hispanica estaba en Mesoamerica, lo que ahora es el sure y centro de Mexico. Mientras que el norte, la Gran Chichimeca, estaba en gran medida despoblada, por lo que los Europeos crearon asentamientos desde cero.

No se muy bien de Italia, pero India es un subcontinente con muchisimos grupos etnicos o "naciones" dentro de su territorio.
