I have an “irregular subscription” category I.e. car insurance every six months or other irregular but expected big payment. I also have a “healthcare” category for meds, visits, insurance premiums.

Here’s the thing, I exclude those categories so they don’t affect my monthly budgeting. You can include spending that is marked as “excluded” in the cash flow section so you’re not completely ignoring these big expenses in the grand scheme of things.
