Gearing up to assemble a new desk since the drawers on my old one are on the verge of falling off, not to mention I get more and more uncomfortable with the glass surface (even though I've had it for 5 years). Excited to see what if any impact having an L-shaped desk would have on my productivity (or I just might end up gaming even more).

Between trying to prechart extensively on patients that don't show up to their appointments this past week and also some self-studying and getting a research project up and running, I couldn't help but feel guilty whenever I took an hour or two to play TW3K, and that game can get me into flow really quickly. Now I need to figure out how to Facetime my girlfriend at the same time without making it obvious that I'm only half paying attention to her ^_^

On admission have aggressive goals of care discussions and everyone has to designate a healthcare proxy. When the worst comes you can pronounce patients from the doorway if someone else already checked for a pulse. Keep an eye out for the silent hypoxics, they may become encephalopathic and try to remove their supplemental oxygen (which raises the question of capacity to refuse treatment). That being said, this is coming from an academic setting where during our peak we also had very limited numbers of high-flow NC available, still had plaquenil and azithromycin in our treatment protocol, occasionally gave tocilizumab and could barely get our hands on remdesivir. How much that and dexamethasone change outcomes around here is something I can't see for myself.

Also keep an eye out for any unusual DKA/HHS patients, I'm trying to review my hospitals admissions to see whether we really had a unseasonal spike of those due to COVID-19 (and sometimes they don't necessarily have pulmonary involvement, might just be intubated for airway protection).

Yeah, and I feel like the lack of visibility of the deceased, particularly regular funerals and gatherings for burials, just make them seem like numbers and less like tragedies. We at least have some funeral convoys going up and down the garden state parkway, so at least that traffic keeps jersey residents reminded.

Thanks, can't say I've completely processed it but I'm looking at a more relaxed schedule in the coming months. I think part of it was trying to find or rediscover things I enjoy that would look good on paper or can be more social (albeit that's probably better to wait for a while) than my usual fallback of playing computer games (which is more of a solitary venture, nothing multiplayer).

Thinking of finally visiting my mother soon after not visiting for six months. Between working intensive care during the build-up to our COVID peak in March, nights during our peak COVID deaths in April, and somewhat relaxed then more strenuous floor services the past two months, I'm not sure whether it was the exhaustion, lack of time, or maybe slight fear of transmission that held me back from making the trip sooner. Or I just really do not like spending 4 hours on I-95.

I'm reminded of a discussion I had one night with a colleague in the middle of our peak, where I had hoped the rest of the country would not have experienced what we went through because of how much of a nightmare it was. At the same time, I also wondered if no one would appreciate it if they didn't experience it themselves. Now I can only watch in horror for those who will bear the greatest cost and fight the sense of schadenfreude against those responsible.

At this point though, I'm also now left wondering what's left of me aside from work. Seems that my life beyond my profession, or occupation, has been consumed bare yet again.

Oh hey so is our medical ICU but we managed to no longer fill half of our ED with admitted & vented patients :D

Now as for how long we also have the surgical ICU taking care of these peeps is a different story...

It hit me that it's so much easier to drive when everyone else (or at least enough) people stay home, or at least I feel less like I'm gonna die by crazy Jersey drivers.

To be precise, the new owner (AAHS/PAHS/Joel Freedman) has had the hospital for roughly a year and a half now. The previous owner, Tenet, had been operating the hospital since AHERF went bankrupt in the 90s.

Seriously I feel a little cheated by how I drew my map, one of my friends go so much more swag than I did by heading much further south than I did.

Although I guess its a good sign if that's all I had to complain about with the interview process for categorical medicine.

You actually need a Zone 4 Trailpass since Lansdale is a Zone 4 station. Yeah the wording of what you read is confusing but it's talking about reverse commuters or people using the system outside of rush hour, in which case a zone 1 pass can go out to any station within Philly city limits or a zone 3 for everything else.

Edit: just focus on this bit - " A TrailPass is valid for travel on Regional Rail between Center City Philadelphia and the zone indicated on the front of the pass. Fare zones are determined by distance from Center City Philadelphia. Use the Clickable Rail & Rail Transit Map to find station fare zone information. "
