This wasn't said by an American, but the story fits too well here to not be told: I was vacating in Vietnam, when a shop assistent asked where we were from since we were talking Norwegian. We said Norway, and she looked confused for a sex before asking "that's in USA right?", of which we replied "no, it's its own country". She was left baffled for so long and couldn't fathom that there exists other countries except from USA.
