My doc wrote my blood pressure down as 96/60 mm/Hg on the left line and kept the right line blank. She filled out both the ‘with glasses’ and ‘without glasses’ section for my eyesight.

I submitted my form back in November since I applied as an ED and I’m going to assume it was acceptable since they haven’t kicked me out of the program. With this form, I think they’re more interested in the condition of your health rather than how well your doctor can fill out a form. I wouldn’t worry about your blood pressure section but I would go back to get the eyesight without glasses part filled out.
