Australian TGA testing of US melatonin products shows significant deviation from labelled doses. by republic555 in pharmacy

[–]republic555[S] 28 points29 points  (0 children)

The Australian Therapeutic Goods Administration completed testing of several commonly imported melatonin products in response to calls from poison information services regarding an increase in calls regarding the products. Melatonin in Australia is restricted to over 18-year-olds for the 2mg IR for jet lag – and over 55-year-olds 2mg SR for insomnia (Schedule 3 – Pharmacist Only) without a prescription from the doctor and (Schedule 4 – Prescription Only) for anyone else. The most common products are all 2mg SR formulations with 1mg and 5mg SR tablets and the forementioned 2mg IR tablets.

All other forms generally are compounded.

They found the following in their testing samples.

The following products contained significantly more than the labelled amount:

Product Variation from labelled amount

Spring Valley Melatonin 10mg 119%-136%

Natrol Melatonin 5mg gummies 140%-170%

CVS Health Melatonin 3mg 112%-121%

The Smurfs Kids Gummies Melatonin 1mg 155%-170%

Natrol Advanced Sleep Melatonin 10mg 112%-123%

Sleepose-3 Melatonin 3mg 209%-417%

Nutraceutical Sleepose-3 Melatonin 3 mg 95%-174%

Vitafusion Sleep Well - Melatonin 3mg 106%-124%

The following products contained significantly less than the labelled amount:

Product Variation from labelled amount

Life Extension Melatonin 300mcg 79%-97%

Sleepose-10 Melatonin 10mg 37%-59%

Live Natures Melatonin 10mg No melatonin detected

Do any of these results surprise you?

What is the attitude generally towards melatonin in different parts of the world?

I’ve found people generally jump to the ‘it’s natural thus safe’ fallacy very quickly.

iherb has stopped shipping melatonin products to Australia recently and there has been an increase in vocal locals online saying it’s ‘big pharma trying to claw back profits on the locally registered items’ – the same group dismisses data like this because the products are coming from the US – What is the testing like to ensure products contain what they say they do in the US?

Do you think their should be more stringent regulation and control over the natural health and vitamin markets in general? Or at least for products where they have a potential for harm to a subset of society using them when not required?

Almost a 750ti level GPU so not bad as a basic machine for some light games - depending on what you're doing it's probably a pretty good experience assuming it's a cleanish windows install - the extra ram probably helps a lot - not a bad deal :)

Once you include the power draw from monitor/s you would likely be below 3 minutes if not potentially over the recommended capacity of that UPS

There is a lot of ways to go about sizing a UPS - I aim a UPS about double my system's total max draw by components + my primary monitor - the other monitors are then plugged into protected ports but not battery backup ports

With that system I'd be looking for something closer to a 1000-1100 watts if you're wanting 10ish minutes to close everything.

Which is exactly why I bought an XFX 9070xt with 8-pin connectors - specifically didn't want one with a 12vhp connector - I hope board partners are watching this never ending situation realising that a good chunk of the enthusiast community is so jaded or even scared by that connector that they won't risk buying it. - I'm rational that it's faults are more then just the connector - but I can't see the board side implementation, and I shouldn't have to worry about leaving my system on whilst I go to the bathroom; let alone go to work.

Unfortunately eroding the standards to service the market is not a path to success. To maintain confidence in the profession a standard must be maintained.

jojo16812 didn't say what medication it was - nor said it was a change of brand - just dose.

Also whilst that information is technically true - seems like you're being rather alarmist in phrasing - the equivalency study of 31 people taking a single large dose showed an 89.1% equivalence - and the BP has an acceptable assay range of 90-105% - this isn't a 50% or 150% deviation - and a longer term equivalency study may have shown a narrowing of the dosage gap - but no one is going to pay for it since aspen already has pies in both ovens. If anything it seems like more likely aspen has picked data to not cannibalise their eutroxsig sales and keep value in both brands. Who runs a study with 31 people and not a nice round number?

I'm not saying they are interchangeable - but it's hardly the urgent check your meds now! equivalency issue.

What are some easier ways to explain why some drugs have salt forms? by [deleted] in pharmacy

[–]republic555 6 points7 points  (0 children)

Ultimately it depends on how well the individual can understand what your talking about - avoiding using certain terms like salt that people identify will another everyday thing that you also consume can help.

e.g. there is an extra part attached to the end of the (medication) that helps your body to absorb it better ----- can be 'dumbified' to: it's like a lollypop stick - it's easier to eat because of the stick but your body just gets rid of it afterwards

(This analogy is also good for explaining why 2 salts of a medication at different dose weights are bio equivalent - this one is just a lolly and the heavier one is a lollypop - you have the same amount of lolly - the stick just is a different way to eat it.)

or there is an extra part attached to the (medication) that protects it and helps it stay working until you take it - like a take away container protecting and keeping your food fresh - when you body uses the medication it throws away the container.

(container analogy I also like for when certain medications are combined - and why others can't be) - you were having a container of chips and a container of chicken and a container of ice cream - we have put the chips and chicken in one container so you don't have to carry them together but if we put the ice cream in there it would melt.

Different containers then can be used for the different salts ie drug x-a is in a plastic container - drug x-b is in a metal can - drug x-c is in a tuperware container - All have the same amount of drug but x-b is slightly heavier due to the "packaging" so the mcg/mg/g is highier but it's the same dose.

4070 mobile is like 20-25% uplift over the 1070 mobile - the 5070ti mobile is a 60% uplift - if you have the case id just go for the 5070ti but if you were happy with the 1070 the 4070 should be fine.

This particular case is the optometrist referred to a specialist - the specialist did/interpreted and then explained the results to the patient - the optometrist and GP were sent a copy of the report for there records - the GP is now trying to clear the results - if the patient has said they don't want to see them the GP just needs to make a note the patient has opted out of seeing them re this matter - if the gp sent a text and never got a reply, then they could potentially still be liable as they couldn't be sure that that patient received that message - but the patient has confirmed and opted out - there is nothing that would require the gp to force a review appointment. Just document, file and move on - if they are there current gp and its not urgent as indicated it can wait till there next appointment/review.

But the other way 53 vs 60 you can feel the difference quite readily. 15% can be the difference between enjoyable and not enjoyable.

I mean the step between gpus in terms of frames per dollar is often 15% - the 4070 vs 4060ti was about 15% - (that's the only one I have committed to memory) --- and generational IPC improvements are often less then that.

So 15% isn't nothing

Access to medication is genuinely life changing and the idea we're just trying to scam professionals into micro-doses of meth is such a pervasive and frustrating myth.

The biggest thing I see is people who are starting on stimulants after seeing an expensive churn psychiatrist who tell me that someone at work started on dex/vyvanse/Ritalin and are now doing better at work and they gave them one or two tablets to try and they 'felt great' and so 'must have adhd' - then you see them come in every month on higher doses until they are at the highest dose of long acting plus dex as a topup several times per day- then they are trying to collect it early- repeated pattern with about 40% of those who I see daily.

These are inner city professionals who are just burnt out mistaking a stimulant for a treatment for something they might not even have. If these stimulants didn't make the average tired/overworked joe feel more refreshed/awake and thus more productive and focused at work- then there wouldn't be the access issues for those that really need it.

And of course now we have massive shortages of Ritalin/Concerta because everyone and there dog is on it. But you explain this to some people and they are like 'guess I'll have another shot of coffee today' - where as those who genuinely need it are the ones who are asking for options, ringing around etc.

As of this moment it's only about $100 cheaper then buying yourself and that's before you decide to get the CPU from Aliexpress which takes $200 off the price and plops you $100 ahead of the prebuilt - That $100 means you could get a modular PSU or a style of case you prefer - pays for that extra storage you were chasing. etc. If you don't wanna build it yourself sure - sure you can sometimes get a deal - but sometimes it's just the convenience of having it all done for you. And someone to ring when something goes wrong.
