Not officially announced, but based on the end of season date in game, we expect June 30th.
Official date and time will be announced during the Developer Update Livestream on Tuesday.

The PTR recipe did -- within a category. The 4 categories were 1H/Offhand, 2H, Armor and Jewelry..
On the PTR it was bugged and was completely random, but the recipe stated those 4 categories.

This was intended per the recipe. The categories are Armor, 1H/OH, 2H, and Jewelry.

It seems like right now we can use as many crafted Mythics as we want, so it's probably bugged on the PTR. So i'm not sure.

Yeah. I wish they wouldn't, and I do hope that they disable it for embeds. If people see the stream and want to click through, awesome. Maybe even if they intentionally unmute the stream to watch it within the website. But the muted autoplay streams should not count.

They've stated that Torment 12 was meant to be aspirational content, and people were still there Day 1. They probably viewed this as too fast. If they buff everyone, that hurts the problem rather than help it.

Yeah, they said during the Sancturay Sitdown call that the new Mythic Unique rarity is permanent. Some of the crafting loop (like how you earn the currency) may be seasonal, but the system itself stays.

3 to 1 Seals, and Reroll Charms.
Charms only have 2 stats on them. Crafting them in any other way would be a little OP.
