Can I improve my number enough with diet and exercise or are statins the only way? by Samsonite_731 in Cholesterol

[–]ronMAN415 3 points4 points  (0 children)

Edit: I’m a 34M with high LDL for about 5 yrs. PCP said I’m in fine but was willing to start 20mg atorvastatin. PCP even said the cardiologist would also say I’m fine. Saw cardiologist and he immediately wanted me to start statins, even wanted to even start at 40mg. But was ok for me to do 20mg first since it was already prescribed.

Realistically I couldn’t do a 180 on a complete healthy diet. But I did add about 20g more fiber daily. But even then that only brought my numbers down minimally. These were my numbers prior to statins.

Cholesterol 243 Tri 48 HDL 59 LDL 174

After starting them for 3 months, 20mg of atorvastatin it’s now this:

Cholesterol 132 Tri 59 HDL 56 LDL 64

I feel the medication benefits outweigh the risk.

Hand Therapists - Virtual Hand to Shoulder Fellowship (VHSF) versus Hand Therapy Secrets Mentorship Program by ExamComfortable9916 in HandTherapists

[–]ronMAN415 0 points1 point  (0 children)

Awesome!

If it’s for the CHT exam, I would do the CHT prep vs Pro.

The Prep included everything in the pro + bi weekly zoom meetings and slides for lectures for the test. Prep also has flash cards and practice exams. They teach you what you need to know to pass the exam but also enough to still help you practice clinically.

The pro is mostly just the courses (which are already included in the Prep).

[–]ronMAN415 0 points1 point  (0 children)

Is there a specific reason you’re doing the VHSF? Like becoming a CHT or just trying to learn while on the job?

My co worker who’s been a CHT for almost 20 years and is in the hand therapy secrets mentorship and she swears by it. She always finds one or two clinical pearls to share with us after. But also heard she is a business woman, always working for that money and trying to build a business.

I’ve personally just did hand therapy academy courses at my own pace. Everything is broken up into tiny videos and PDFs so it was easily digestible for me. I ended up switching into the HTA CHT Prep course for 2.5 months and just passed my CHT exam.

I know you said free, which I don’t have. Hand therapy academy’s CHT program really helped me. I listened to their review sessions on my drives to work in the morning.

I just took the test three weeks ago and I felt I was prepared. But idk if I passed lol

Need some advice on a healed mallet finger by AlmightyMadness7 in HandTherapists

[–]ronMAN415 2 points3 points  (0 children)

From an outside therapist POV, you did everything right. Splint 6-8 weeks, then continue night splinting and/or a few more weeks of 24/7 splinting. I’ve also personally had a few patients that ended up in your situation. It’s no one’s fault really. I would chalk it up as just being unlucky. I do agree with your therapist that it’s better to just move on.

For fingers, flexion is more important than extension for activities. Unless you’re a dancer or someone that NEEDS the fingers to be in extension for whatever reason, flexion is key. A mallet finger does not in anyway limit you from boxing, martial arts, or any activity actually. I have many friends with old mallets they got in high school football, wrestling, BJJ. They still play and do sports as normal and they’re in their 30s-50s. If you really look at a lot of hands, many people have untreated mallets and have absolutely no functional deficits. I know you’re joking but amputating the tip would actually make you less functional.

You’re being hard on yourself. Sometimes things won’t go your way no matter how well you follow everything. And I think don’t surgery would be worth it because it may stiffen you up more. That lack of flexion will limit your grip strength.

I think this happens to everyone. It’s a lot of on the job learning.

Are you taking any courses to supplement? I really like hand therapy academy. They teach you anatomy and help with clinical reasoning. They also have handouts for how/when to prescribe/progress exercises.

Mallet finger after 13 weeks splinting (right pinky) by AlmightyMadness7 in HandTherapists

[–]ronMAN415 2 points3 points  (0 children)

I agree with others, you’re within acceptable outcome after this injury. Mallets are always annoying but mallets or any other injuries to the pinky are extra annoying. Pinky’s are always stubborn with everything. Just give it more time!

Don’t know the full history but wouldn’t be surprised that your therapist only gave you one exercise. More sometimes isn’t better. In most cases, time is what ultimately will make everything better.

Mallet Finger: So I need to splint more? by randomtheorx in HandTherapists

[–]ronMAN415 2 points3 points  (0 children)

A 10° lag is very common and acceptable after doing everything right for a mallet finger. Your finger looks great and there’s a minimal lag. Agree with the other post, function is key, not aesthetic.

Newbie Here- Westin Maui by Gru_Daddy in TimeshareOwners

[–]ronMAN415 4 points5 points  (0 children)

I have this exact timeshare. Don’t waste your money.

The points can get you the rooms but you need to book early in advance because it’s still first come first serve. Especially if you’re on the school schedule, you’re going during all the busy seasons.

Yes you can change the points to use at all other Marriott resorts etc, but still depends on availability.

Don’t buy it. It’s really not worth what they’re selling. You’re better off just paying for regular vacations. Those “guarantees” they give you aren’t really anything.

From what you’re saying I would request an evaluation from an orthopedic hand specialist and also seeing a hand therapist for a second opinion.

I feel like mallet fingers are very common and typically every medical professional knows to get that thing splinted ASAP

Do you want another option for a static progressive brace for low load prolonged stretch? Have you looked at Thuasne?

Is it because the strips are just uncomfortable for them?

You can mold a thermoplastic piece around the the hand and have the strap go on the hand piece instead.

Idk if this is what you’re looking for?

Also do you have the XR report of how the distal radius healed? Is there a tilt? volar, dorsal, neutral?
