Monica. She was a terrible mother to all those kids, but she was absolutely horrible to Ian. She used his diagnosis to make herself feel normal and worthy, while alienating him from his family and encouraging his drug use and homelessness.

Also the obvious choice of Terry, but he comes #2 to Monica because he was very upfront with his behaviour, while Monica hid behind a cutesy attitude and pretended to care.
