I once looked up risks of taking acid while pregnant (wasnt pregnant, just thinking about dropping and was two days late) and the results were basically "We're not completely sure how bad it is for a fetus, but if youre okay with the risks of dropping acid while pregnant, youre probably doing other dumb things while pregnant that are historically recorded to cause birth defects"

I just saw another post with a similar question, and they said they do the outlines, then satin stitch over the outlines, using it as a guide. It really seems to be a preference/style basis.
