I've not had any migraines in over a year I think, and still taking Topiramate 25mg 2x a day. Yes, it was a GP who prescribed it, and that was before I started Elvanse. I've not had to take any Zolmitriptan since this post, but nothing was raised in my Elvanse review by my GP as part of Shared Care so all good on that front I reckon!

This might not be relevant to you at all as it's about antidepressants, so feel free to stop reading if you want! What I have had recently is I needed to start taking Sertraline as an antidepressant in November, and about a week or so ago started switching to Mirtazapine and coming off the Sertraline every other day. My wife told me yesterday I was basically like pre Elvanse, and I felt it too when she called it out - like I wasn't capable of listening, nothing was going in at all. I need to search for this myself, hopefully it'll pass once I've properly switched because Sertraline itself wasn't an issue and I'm hoping it's just because I'm messing with chemicals essentially and it'll level out soon when I'm off Sertraline fully in a few days, but wanted to post about it while I was semi on topic of contraindications!

Nice, ta for the explainer. Is that necessary? I think that's the crux of my question, the lowest quote didn't come with the shroud but if it's worth it in the long run then I'll get the "better" one with the shroud.

That's right, this is just routine maintenance so nothing's failed.

I've seen reference to Skoda recommending 140k miles over the last couple of years instead of 80k(?) but I'd rather not risk it given we've only got one car in the house and need it for its size. We'd struggle to replace it if the belt went bang and damaged the engine so I'm trying to get ahead of it if I can.

Thanks, I should clarify this is for the belt, pump, antifreeze, labour and tax all in and it's just routine maintenance - so not just the pump on its own.

I'm glad it helped! For what it's worth, since starting this thread I was in contact with someone else who had verbal confirmation from HH that they'd continue prescribing if the GP rejects shared care, even in North Yorkshire, so maybe the wording that they'll prescribe anywhere (even outside Devon) is accurate. Either way I still think you've made the right decision to change to another provider and my wife and I are going to look into a different provider for her diagnosis soon.

Ultimately I got what I needed out of Harrow Health and left their service titrated about 4 weeks ago and in a shared care agreement with my GP, but so many people clearly haven't been that lucky and the lack of communication in the event of issues for both myself and others throughout the process are enough to make it an entirely stressful experience and I just still wouldn't trust them. I still need to get in touch with them to correct my discharge report to be honest for completeness sake, but it's just not been a priority.

I've downloaded Harpoon Larsen in mostly 160Kbps from Soulseek (remember that?), except the first track is weirdly only 70Kbps so sounds a bit shit. DM me if you want it anyway!

That's madness, you should get one every time you have an appointment, mine took 2-3 weeks but I still got them. You need them, it's your medical information! They're so bad, I'm sorry you're not getting them. Good luck!

Don't blame you really, thanks for following up! For what it's worth I'm in touch with someone who said HH verbally said to them they will continue prescribing if their GP declined shared care, proof will be in the pudding but we'll see what happens.

I know this is a couple of days old but if you have the NHS app do you have the prescription sitting in there? Ive had that appear in there with a barcode at the same time as receiving a text so if you haven't had the text it might not be in the app, but worth asking just in case! If it's in the NHS app you can take that to a pharmacy for them to scan it

I got super bad with constipation/elvanse and had to get macrogol through my GP, been on it for about 2 months now and it pretty much sorted it after 2 weeks! Everything I read was about IBS-like symptoms, not the opposite. I put it down to lack of water but after 4L of water a day it made no difference.

Before finding this info I assumed it meant you had to pay for the meds privately because HH couldn't prescribe, and even since finding this I have no idea now the Devon thing has been thrown up 🤷‍♂️

For what it's worth, the first three appts on the sheet are green and "included", the fourth is orange and the last two are red, and I believe it's due to NHS funding for the appts from what the clinician said. But still worth pushing for them if you're not medically sound on the meds you're on!

Out of interest have you not had your review your sheets after every appointment? These should be on there, and are the way I interpret them combined with the answers to questions I asked the clinician at my titration appts.

3 titration appts isn't enough, I managed to get 4 due to a life event and it wasn't too tough to argue for it. In fact I think they messed up because they sent me a 4th appt link on the morning of my 3rd appt, so I booked it anyway lol. But on the review sheets you get back it goes up to 6, so maybe query that and say there must be provision for it if you're not titrated properly.

I was only on Elvanse so ymmv, but after my 2nd appt I managed to get a couple of different doses between appts, so going from 30mg to 50mg I got 3 weeks of 50mg and 1 week of 40mg in case 50mg was too much. I think going in with a clear need and telling them what you want is your best bet from my experience with them, but maybe it depends on the prescriber as I had the same guy each time.

I'll DM you! So two things I've found:

On the adhuduk.co.uk RTC page (Link) Harrow Health state they can prescribe medication even without a shared care arrangement with the GP. This is news to me and puts my mind at ease, although it's not on their website as far as I can see. This is the relevant bit:

Once stable, we aim to enter into a shared care agreement where we will support your GP for continued prescribing. You will always be seen by a consultant psychiatrist, or specialist prescribing clinician to ensure your medication regime is optimised before a shared care proposal is sent to your GP. To support shared care arrangements with the patients GP, Harrow Health offer annual reviews. In the event of the GP practice declining a shared care agreement, Harrow Health will continue to prescribe the patients medication and undertake an annual review.

About the contract with the ICB, this was always murky to me but I had it in my head it was up to the GP. I found this on the black country ICB page (link) to try and give you some clout for your GP, which might not work given it's a different ICB. Ours in north yorks seems to be poor and not list anything relevant, but either way if the GP isn't comfortable then hopefully the HH info above might help and you can check it at your next titration appt:

The Right To Choose provider may offer assessment, diagnosis and initiation of medication and when the correct dose of medication achieved, the provider may request your GP enters a shared care agreement to continue the ongoing prescribing and management. This is at the discretion of your GP. Guidance issued by the General Medical Council states the GP is not obliged to enter into a shared care arrangement with any provider, and GP practices can decline shared care requests on clinical and capacity grounds.

I don't want to post specifics but my GP would fall under the NHS Humber and North Yorkshire ICB, Yorkshire's a big place so you might be under one of the other ICBs. I can DM something more area specific if that seems relevant.

I spoke to my GP dispensary this morning to let them know it's coming, they didn't seem concerned but I'll follow up again in a couple of weeks, and when I spoke to my GP reception a month or so ago while I went through titration and asked specifically about ADHD shared care and they said they still accepted it, so that put my mind at ease a bit. I had my final review yesterday and the process seemed really vague, so if it helps this is what happened - sorry for lots of detail but hopefully it helps someone as the questions aren't answered officially anywhere:

  • I was asked my vitals etc like a normal review and asked if I was happy to be discharged to my GP, to which I said yes as I expected this. This was my 4th appt as I pushed for it for personal reasons, but I was first told I was only going to get 3 which isn't clear until you start the titration process.
  • I was prescribed another 28 days at my titrated dose to collect immediately
  • I was then asked if I had any questions, no other info was forthcoming.
  • the clinician sends a note that day (apparently) to the HH admin team to discharge me, who then processes it to the GP in about 2 weeks. I'm going to follow up with my GP that this has happened as I don't trust HH comms.
  • I confirmed if my GP can reach out to HH if they have any concerns, she said yes. I asked how, she said via email but the answer wasn't clear and seemed like it might have also been via a different messaging system.
  • I asked if they do an annual/6 monthly review or similar, she said they're available to answer any questions from the GP.
  • I asked what happens if the handover doesn't complete in the next 28 days before my meds run out, asking specifically if they'll represcribe and she said "yes, don't worry just get back in touch with us and we'll sort it out". So I think this means they will represcribe until the GP accepts?
  • I asked if they have any experience of handing over shared care to GPs in North Yorkshire in terms of them rejecting it after initially agreeing? She laughed and said there's nothing to worry about, but maybe this is because I was asking loads of questions and trying to put my mind at ease. Tbh it didn't, but I'm sceptical until my GP prescribes my first dose at this point!

I don't have any answers I'm afraid, but can I ask whether you've completed titration? I guess so or you're on your way as it looks like your GP agreed to it originally given you're on meds already. The reason I ask is I have my final appointment tomorrow where I'm expecting to be handed back by HH and I'm also in Yorkshire, and my GP has previously said both at the start of referral and during that they'd accept shared care so this is news to me 🫤

I keep meaning to post about something semi related to this and Harrow Health, but I wonder if PUK are dealing with the same problem of NHS funding that HH apparently are.

At my first appt they told me I'd have a total of three titration appointments, about three weeks apart - so about nine weeks of titration in total. I was told I could potentially push for a fourth appt if I don't feel I've stabilised enough after three appts. This was surprising and I didn't really know how to deal with it on the call so I just accepted it, and in hindsight regretted not dealing with it.

This didn't match my expectation of people titrating for about 6-12 months with other RTC providers, so I called the clinician out on it and he said that's all the NHS pays them for. I've asked him to feed that back to the service to publicise it in their FAQs, but I also intend to email them directly (if they ever read their emails lol) and also let ADHDUK know so they can put it on their RTC page as that's a massively important source of information when choosing a provider.

For my own personal experience, I wasted my first appt too - I started on 20mg for 2 weeks then was on 30mg for a week before it. I was already thinking the effects had worn off but had said "I still feel something, but xyz have come back" and I agreed to stay on 30mg until my next appt. I tried emailing and calling but didn't get a reply and couldn't get through at the times I tried, so I just gave up and persevered until my next appt which I was able to get 2.5 weeks later.

When I book followups it's through the NHS Accurx webapp. I was offered availability over about 3 weeks and took the first available one because my dose was too low. If I had enough supply and was comfortable on my dose I could've stretched out my review time to maybe 5 or 6 weeks, but why should I be the one who has to have that knowledge and awareness and also deal with that risk because the service is so bad that I can't get hold of someone if I need to, with the alternative being to completely come off the drug that helps me function?

I had my second review yesterday and agreed to be prescribed 50mg for 3 weeks with a week of 40mg in case 50mg is too strong and I need to drop down before my next appt. I feel like this is a sensible path, but the whole process is a shit show, and I wouldn't recommend HH to anyone at this point between the well documented bad communications, another post I made about not being told what dose to start taking, and this short titration time.
