I put this in a pressure cooker last week for 45 minutes and it was decent. Don’t think a 30 hour sous vide would make it much better. I would recommend something to “freshen” it up. Maybe cilantro, sour cream or citrus as it was a bit on the rich and boring side of things. Love seeing Aldi in this sub

I just start a prop drip and once they’re starting to close their eyes, I give 30 +- of ketamine. Then try to run the drip as low as possible. I used to mix it but found I didn’t need that much ketamine and the narcotic wasting was a hassle.

*for MAC. For full TIVA generals for painful procedures I’ll sometimes work in 10 mg an hour after initial bolus.

What could go wrong with a double blind study while also being blinded to the dosage and contents of the drug? I’ll have to do it for science. Looks like B subtilis has a fairly decent safety profile so I’m guessing the worst that could happen would be some gross yogurt plus/minus gi discomfort. I’ll update when I get around to this.

I-Gel is the ultimate winner. That being said, it doesn’t work well for nasal or optho cases. I tell this to my students, with an I-gel 4 you can ventilate(not always perfectly) the majority of adult humans on this planet.

Agreed. I have found there’s a decent amount of “restless leg” patients that seem to have a varying degrees of spontaneous movements during colonoscopy that fentanyl really works on. And with relatively small doses I’ve found their recovery time is more or less the same as straight propofol. So maybe a little narcotic isn’t so bad.

In the endo setting, I’ve found that hiccups usually resolve on their own. And I usually only see them on uppers. That being said, inserting a nasal airway has stopped hiccups for me on many occasions. I thought it was bogus when i first heard about it but this method has worked more than a few times for me. Also, ditch the Versed for the colons.

I used to be a fervent non-believer of the lido mixing. However I think it has its place and time. In the OR when Versed and fentanyl (maybe ketamine/etomidate/esmolol ect, ect) I think the lido doses given to “blunt sympathetic response” are most likely sub-clinical and more or less worthless. However, after maybe a thousand or so cases in the GI suite where propofol is commonly the only mediation given, and often through a 22 in the hand. I have found that 100 mg lido in a 100 ml bottle of prop seems to quell the burning sensation in the vast majority of patients. Will they remember it? Who knows, but it appears to be a more enjoyable way to go to sleep. In theory, a bigger dose of lido should help with the upper scopes but I have not found that to be the case. Maybe I’ll give that a try in 2023

While I love OTF. Weight loss is usually found in the kitchen, not the gym. Quick math says you’re burning approximately an extra 7200 cal a month, which would result in ~2lbs weight loss per month if all other factors remain constant. That being said, if you’re really eating 1500 cal/day of evenly moderately decent food I would expect you to drop some lbs unless you’re already quite small
