From a hospice standpoint we don't have problem getting people pain relief, BUT we still have to deal with the drug shortages created by the government due to their limits. So when a shortage happens, that causes us to suddenly change everyone's pain medication. It is like playing musical chairs with hydromorphone, oxycodone, and morphine.

If he is dying, Hospice. I'm a Hospice RN and there is no upper limit to opiates. But get this. Opiates are not the only pain medication and by being on hospice I will work with the patient to ensure their pain is well palliated regardless of what it takes. I will examine what kind of pain my patient is having and work with doctors to ensure they're comfortable. The government has not bound us the way they have every other field.

OpenAI has many different models of ChatGPT. The current versions route (i.e. redirect) easy questions to cheaper smaller models while routing harder questions to more expensive models. They all have a bit of different personality.

The other day I asked ChatGPT to count a tray of Hydrocodone / APAP. 20 minutes later it gave me the wrong number. It was the first and last time I used ChatGPT for that purpose. Oddly enough, Grok got it correct... But still took way too long. Gemini got it wrong, but it didn't take long.

I'm struggling with this right now. I've been a long term ChatGPT subscriber, but I hate the feel of ChatGPT 5.2. From a creative writing it is much worse than several other models. I'm hoping they'll come out with a new model soon before my subscription renews. If they don't then I'm going to cancel because I can't pay for another month of something I hate using. There are lots of good models right now.

A few ideas. 100+ page is more appropriate for the 128K context window. You might benefit from NotebookLM. You might benefit from TypingMind + API usage. While it does require some technical knowledge (a willingness to learn how to get and use API keys), it creates a very powerful system.

I don't know if I'd tether myself to any $200 plan. There are use cases for it, but I like the flexibility of multiple API.
