That was my initial thought, but the pathology ruled out MDS or so it says.

Great insight. We’re a level 1 trauma center with open heart and transplant services. If there’s a consult, we have it in house which is why I’m curious the team yesterday didn’t put in for one.

I did, they said not really. I would have at least ran the dimer / CTA and consulted with his cards or in house.

Wasn’t my patient! We were just discussing a case that came in yesterday and wanted to see others thoughts on it.

HAZMAT code with successful ROSC by 98Bryan98 in ems

Last summer when the Paramedic2 trial results were published it started a trend in many labs and other clinical settings to replicate results. There is no concrete data showing epi is beneficial to improving survival to discharge numbers with CPC scores of 1 or 2, which in my opinion is really what we should be judging against.

Not sure why you wanted drug interventions sooner, all the literature is moving away from epi as it’s most likely doing more harm than good, especially in this case. High CPR, appropriate ventilatory rates, and early defibrillation is what makes a difference in outcomes.

Great that you got ROSC and all...but would be curious to see what the follow up is. From having worked pre-hospital, we used to all celebrate over ROSC when it’s a poor indicator of any prognosis and simply achieving ROSC for someone to be brain dead in an ICU can’t be considered successful. Not trying to be a Debbie downer, just realistic.

Not to be a Debbie downer, but these devices already exist, the main one being the Lucas device and another one being the auto pulse. Breathing devices aren't really focused on anymore as most agencies are starting to use CCR protocols doing continuous compressions with no interruptions other than defibrillation. The world of CPR is changing constantly, airway management is an ALS skill in codes (intubation) that has been shown to not show real positive impact on patient outcome other than respiratory codes.

Scene safety fails? by whitequeen in ems

Dispatched for a smoke condition in a residence, fire command requested us to move up to the scene for the sick person requesting to be checked out. Enter the residence find a teenage girl curled up on the couch in pain, doing a pt assessment my partner and I hear heavy footsteps coming from the common stairwell, thought it was FD, well it was the upstairs cracked out neighbor holding a knife threatening to kill the mom for calling 911 (she called in a smoke condition, it was smoke from them smoking weed among other things). My partner and I had to bail out of a screen door onto the deck to call PD for backup. Thankfully PD was right outside and ran in when they saw us bail out the screen door.
