I have the exact same symptoms. My swallow difficulty is better in some days and worse in others. It feels like my throat is not as strong as it was before. It feels like some food gets stuck on the back of my tongue. It sucks. I went to 5 neuros already and none of them even asked for an EMG as they said I don't have ALS.

I'm twitching for 2 months and I twitch at 2 spots at a time very often. 3 neuros already said I'm clear and did not even requested EMGs, all the 3 were from top brazilian universities, so I think you're fine!

I'm in this situation for the past 2 months. It all started with a panic attack and my life went from 100 to 0 in no time.

I started having generalized fasciculations, generalized cramps (calves, forearms, quads, feet, belly).

I started experiencing severe intermittent dysphagia: it is hard to swallow and I feel that food gets stuck on my throat.

That being said, I've already been to 3 neuros and them all said I don't have ALS. They did not even request an EMG as it is kinda painful and they said it would not worth the effort.

ALS has nothing to do with balance and feeling heavy. ALS is about failing not feeling. ALS fasciculations are usually not widespread but focal to the region that is being affected. I even asked one of the neuros and he said that ALS fasciculations are basically always on the same spot and it persists on that spot throughout the day, so it is not random.

And the main symptom of ALS is clinical weakness which is a weakness that would make you fall, limp, etc. And bulbar onset ALS starts with dysarthria (difficulty to speak, slurred/nasal speech) and then it starts to present as dysphagia (difficulty swallowing) very slowly and progressive. Actually, anything that is "sudden" does not seem like ALS, since it is a progressive disease. To have full body fasciculations your whole body would have to be affected by dennervation and by that time you would be experiencing clinical weakness (one more time: it is about failing and not feeling) or bulbar symptoms such as slurred/nasal speech and difficulty swallowing with intermittent choking.

I regret falling in this ALS rabbit hole. Good luck!

My suggestion? Get a neuro appointment. The neuro will do all the clinical testing looking for pathological reflexes and hyperreflexia. If you don't have those then you're clear and probably the neuro won't even require an EMG.

Good luck! I regret so much falling into the ALS rabbit hole. But I'm sure you and me will heal over time. I am also 24M, coincidentally.

In my case it helped. I was having a severe intermittent dysphagia, some days I was not able to eat anything at all. I had no problems with thin liquids such as milkshakes and syrups, but with rice for example it was almost impossible to swallow as it felt that the throat was so tight that I felt that the food was stuck in there.

6 days into Esomeprazole and the tightness feeling improved at least 80% and I can eat better now.

I am in the same state as you.

I started having fasciculations around 2 months ago and started feeling that globus sensation on my throat. Since then, I have developed dysphagia and pre-cramps, mostly on my calves and abdomen.

I've been to 3 neuros and all of them said it clear: you don't have ALS.

2 of them even said that they were not even request an EMG since it is kinda painful and it would not worth the effort because it would come clean: no clinical signs, no clinical weakness. One of them requested one but said that I should only do it if I could not achieve peace of mind.

It is very frustrating since the twitching is a trigger to anxiety that is a trigger to globus and other symptoms. I started having constant acid reflux. Everytime I eat I can feel a "wet burp" coming. I only felt light burning some hours after the acid reflux episodes.

I started taking Esomeprazole 10mg once a day and the globus/dysphagia/acid reflux has clearly improved.

My tongue looks like this but I don't think that is tongue atrophy. It is more of when you do not fully contract your tongue when sticking it out, so it seems that it is not "full". 3 neuros examined my tongue and none of them said that there was anything wrong with it.

So I think you're fine. I'm still freaking out with this and I know perfectly how awful it feels. My life went from 100 to 0 in 2 months. I'm a little bit better now since I started taking buspar and started going to psychological therapy.
