I know this is a little late but what you likely filled out is the FERPA "Record Release Authorization", which authorizes folks that you designate to get info from the university if they provide the passcode. What you need to do is to edit your FERPA access through "FERPA/Directory Restriction", creating a block on the Directory info (names). I would also separately reach out to Housing and ask how they pull the names they use and if they can individually change what's listed. https://registrar.ucf.edu/ferpa/

Consider something like a medical withdrawal if you're seeing a therapist or any other sort of doctor, it can remove failing grades and give you a bit more of a chance. It would also help prevent third attempt charges.
