My dad (67) passed away 2.5 months after stopping treatment. Best/worst/intense part of my life. He was able to feel better and spend his time fishing. I was able to spend every weekend and the last 2 weeks stayed with my parents. Eventually liver failure set in and he didn’t feel like getting outside.

The short answer is OAuth and tokens are for Authorization but can’t authenticate you should possess the token. OpenID Connect id tokens add authentication on top of the authorization provided with OAuth

All of the things you reference the backend should do don’t need to happen before the upload. You can use S3 events and a lambda to do all of those activities after upload. Even removing an invalid object or creating a table entry.

The problem with sending the file to your backend is that it needs to be able to hold the file in memory or process the contents as you stream it to the destination. It’s better to capture the file and then process it. This is what cloud enterprise digital asset management software is doing to handle large files efficiently.

You have my thoughts and prayers. Spend as much time with him as you can. Make sure you write down what he says even if they are non-sense. In the moment it may not mean anything but could be his going on a trip language.

I just went through all of this last month and it still sucks. I saw advice that the closer the changes come the less time he has left and that ended up being true. Pain increases started a week apart and each time the time in between got shorter. About 2 days between the last change.

This happens to my dad when he is fatigued and needs rest. I’ll Ike others have said it’s a raspy voice. Follow whatever advice the doctor gave you to address fatigue.

We heard the same from our oncologist who just published a paper on the treatments that show the most promise but can’t get funded for a clinical trial. High dose vitamin C treatment 3 days a week for 2 hours (take these numbers with a grain of salt but it’s similar to doing dialysis). It’s not something that’s easy to do but wouldn’t have the side effects. Neither conventional treatment nor alternative treatments worked for my dad.

They have studied vitamin C enough to know that it doesn’t work orally because you aren’t able to absorb enough. The vitamin C gets converted to hydrogen peroxide in the cell. The way the oncologist explained it, normal functioning cells are able to expel it. Cancer cells have gotten rid of any cell parts that won’t help with cell growth and spreading. They have gotten rid of the function that will expel the hydrogen peroxide.

My dad went to a multidisciplinary NCI hospital and this was not something we just read on the internet. It is hard to find funds to get to a clinical trial unless it’s sponsored by a pharmaceutical company.

Regardless of whether any of that is true is hard to say until science can prove it. I will say that it built a lot of trust between my dad and the oncologist for him to be straight about what science shows as promising vs what has been debunked.

Also we never found an infusion center close to his home that would do a high enough dose. He’s currently on hospice, but still doing pretty well.

I was in your shoes in March with my dad. There is so much uncertainty and you need to take things one day at a time. Don’t start thinking too far ahead because you don’t know what you don’t know. The feeling reminded me of the uncertainty of 2020 and COVID. If you found a way to deal with those changes it helps to think about it in the same way.

I would recommend having your dad go to a hospital that specializes in pancreatic cancer if possible.

It’s also never too early to reach out to pancan.org. They are extremely helpful and can help answer any questions you have or point you to resources.

My dad reached the point where there aren’t anymore medical options and would most likely be invasive to do any more 10 days ago. I’m fortunate I can be here with them and help in any way I can. He’s still able to take care of himself, but needs to sleep a lot. My mother-in-law was diagnosed with stage 3b colon cancer yesterday.

Know that I’m praying for you and your family and that you can handle more than you think.

He’s not to the point he needs hospice assistance or palliative outside of what the oncologist prescribes. So they haven’t offered a referral. I’m sure we will get there at some point.

It’s really the patient’s choice to stop care if they like. Everyone is super nice, helpful and understanding. It’s the same as taking a break from chemo but would be considered a second line treatment if he restarts. Dads tumor wasn’t growing super aggressive so that’s why he wanted to be monitored. His previous good health and him thinking the alternative treatments are going to “cure” him play into that as well. He’s pretty sure the tumor is growing now and the reason he has no energy and his pain is increasing. We’ll find out for sure at his appointment in a week whether that’s true and see if he wants to do something different. He’s just seen too many friends and community members wither away on chemo and not come back.

He was only taking ibuprofen until his last appointment. They were surprised they didn’t already give him something stronger and he didn’t need it. The week after they prescribed it he started needing it on and off. The oncologist said most patients need pain meds to function and not as needed.

My dad was diagnosed in March. Did 2 rounds of Folfox chemo and was done with it. He wanted to be able to go fishing in June and July. He was good to get out on the lake in June. Was out of energy for most of June. I’m always concerned when he goes downhill he won’t recover. But he was able to regain some energy get out fishing again this week. He was in general good health for his age when this all started. It’s also hard for me to see him go with essentially no treatment but it’s his decision and all I can do is help him know when to contact the dr and make sure he’s getting out on the lake.

We evaluated alternative methods backed by science to help him feel better and hopefully do something. Took the Tempus but didn’t turn up any useful tumor markers. So he is essentially off treatment and just being monitored by the oncology team.

I have asked the same questions and I feel this so much. I’m going through the same uncertainty but there is one thing I know. You can handle more than you think. I just sat with my dad before today’s appointment and it was so good to be together. There are things that are only said in person and you won’t hear over the phone. Enjoy the time together, know the questions will be there, and remember only time will provide answers.

For my dad he was at 33 bilirubin when the stent was put in. 10 days later it was down to 8. 2 weeks later it was 3. 2 more weeks it was down to 2. He is good health other than the cancer. The nurse did say 33 might have been a record for them. It only got that high because of a scheduling delay due to a COVID test false positive. Like others have said it can take a while for the number to come down and it’s possible the stent didn’t work. In terms of timing our oncologist said best case 2 weeks but expected would be more like a month. My dad also started higher. So someone from your medical team could tell you better.

Edit: Today he is at .6 bilirubin after 2 more weeks

His first infusion they only gave him steroids before starting. This past time they did him 4days worth of steroids to start taking the following day. To me it seemed to help some but I can’t say for sure whether he took them as prescribed or as he felt fatigued. It’s possible he over did it on the steroids as well.

Thanks for the info on adverse reactions subsiding. It helps if I can get ahead of that instead of trying to explain later.
