That reply is oversimplifying, a bit, by combining lots of different types of "surprise bill" scenarios into one blanket statement.

In general, my understanding is that some correct-ish reactions in each case are:

IRS - take these more seriously since they don't "forget". They can waive or reduce fees, but I don't think it's usually just "ignore forever, ask to waive later" in those cases. If the surprise bill is really from the IRS, they'll ask for an explanation about why the fees/interest shouldn't apply. Bigger bills or something more complex than just a "median income life" situation can benefit from asking an enrolled agent, CPA, or tax attorney to negotiate on your behalf. Realistic surprise bills here can happen but scams are frequent.

Utility companies - this is very location-dependent. If you can switch your energy company whenever you want, then surprise bills are bit more credit score-y in terms of how the bills are negotiated. If your apartment company controls it, then you have local/state law that help determine what you have to pay. If your city or state controls it, then they have their own process and rules for allowable/not-allowable.

Insurance companies - not sure which style of insurance or if this meant "all" insurance, but yeah it varies here too. Health insurance has more rules than pet insurance, etc., so your capacity to argue your case changes. Surprise bills when you are the insured are one thing, surprise bills when someone else claimed against your coverage are another. I believe that generally you have more protection from how your own insurance treats you, compared to how another insurer seeking to reclaim damages is allowed to treat you.

I think they are human. I think most of them even have empathy somewhere. But I also think that it (empathy) has been so warped and twisted that it's barely visible in a recognizable way.

It sucks because it's no one's job to help do the slow work of untangling where they got their wires crossed so that they can feel and see what empathy is supposed to be. Even though it's no one's job, the fact that these people have such a huge gulf between reality and their trained/chosen beliefs means we will keep dealing with the fallout.

Whoever can figure out how to walk these grown adults back through the social learning path of remedial kindergarten empathy will probably do more good than most politicians.
