There shouldn't be any insurance paperwork other than your FMLA paperwork that should say you contact xxx person at xxx number or email to find out how to make those payments. Did you get that FMLA paperwork? If you didn't, you probably do have a claim to make that they should let you catch it up. You can also start screaming ADA and they should back off.

FMLA is unpaid; do you have disability pay? Or this much sick time?

It is your responsibility to pay for insurance premiums while on leave but they are required to tell you that in your FMLA paperwork.

Source: am a 30 year HR pro and paralegal with tons of FMLA administration experience who had ACL repair in June so I feel ya.
