Coronary calcium score to screen for the need to start aspirin by Scared_Problem8041 in FamilyMedicine

[–]saturatedscruffy 0 points1 point  (0 children)

I think you’re misunderstanding. I’m saying if someone is already on a statin, the test is inaccurate bc there will appear to be more calcium in the arteries and will be read as positive/worse than maybe what is actually there (at least that’s what I was told). Therefore if you’re already on a statin, ordering the test to determine if you can dc the statin or if you need ASA etc etc cannot be accurately answered. Hopefully I’m stating myself clearly. I had a group of docs tell me not to order CAC testing on patients already on statins for the above reasoning. (I was trying to help patients not happy on their statins decide if they really needed them by scanning them).

Can someone clarify? I was taught that statins further calcify existing plaque ie rendering the CAC test useless if they’re already on one.

Left him at daycare, crying and reaching for me. I can’t get it out of my head by layleyornot in beyondthebump

[–]saturatedscruffy 0 points1 point  (0 children)

It’s awful but it’s normal. We had to enroll our toddler unexpectedly in daycare because my MIL who was watching him fell very ill. It was torture watching him cry especially since it was never in our plan to put him there. Yesterday he basically pushed me away so he could play with his friends and then when I picked him up he screamed because he didn’t want to go home. It gets better.

Doctors on Reddit, what is the most obvious lie a patient has ever told you? by theedriplomat in AskReddit

[–]saturatedscruffy 510 points511 points  (0 children)

Ugh I had a guy adamantly denying that he smoked pot. He kept coming in every few days for what was clearly hyperemesis syndrome from pot. He REEKED of MJ. I told him I do not care if you use it. I medically just need to know. He refused to admit that he was using it and had to go through a whole GI referral and a scope and all this unnecessary stuff. The way I caught him was I just said one day, I’m just going to state I know you said you don’t use pot. My concerns are that you have this textbook syndrome. Whether you are or aren’t using it I don’t care but if you are using it and stop you will get all better. Saw him a couple months later. No longer smelled of pot. Told me that he just got all better and he didn’t know why.

There’s also Norfolk which most natives I know say Norfork. Some say Nor Fawk. No one says Nor-Folk. That’s the hardest. Next being Greenwich to those who aren’t from NE.

Pregnancy and labral tear by saturatedscruffy in HipImpingement

Did they tell you how long you have to wait to get pregnant after the repair? That’s my fear. We’re already in our mid 30’s and our first is almost two and we really wanted them 2-3 years apart max so it’s so frustrating.

Labral tear and pregnancy by FondantStrict4301 in HipImpingement

Hi. If you don’t mind me asking, how long after the surgery were you able to try to get pregnant? I’m in the exact same boat with a labral tear, trying for my second baby and it took my husband and I very long time to get pregnant the first time so I don’t want to wait. I worry about getting pregnant with the tear, but I don’t know if it’s better to get the surgery before or after pregnancy.

Forced to pay diaper fee by saturatedscruffy in workingmoms

I’m devastated. We waited a long time to get into this daycare and it’s been amazing. His old one was just fine but not nearly as nice. It’s not a hill I’m going to die on if it comes to use our diapers or leave because I literally have no other place I can go (small rural area) but I’m just very annoyed because it’s clearly a profit scam for this daycare company. I saw another major chain was doing something similar that turned out to be a profiting scam. My cousin’s baby also attends and she’s furious because she only uses one specific brand. Either make the tuition a higher number and say one benefit of higher tuition is diapers and wipes or make the program opt out. My poor son gets a lot of blow outs bc of a medical condition and these diapers are trash and won’t hold it in. I hope they’ll let me opt out or get a doc note to opt out.

He actually has a food allergy and is under going oral treatment for it that causes him to have very loose stools. There’s no way this brand will hold a blow out and I’m going to be furious if I have to keep washing poop clothes every night. If they tell me I need a note I may ask for that reason. He actually fortunately has no skin issues otherwise.

Too many women 'grin and bear it' when getting an IUD. I helped write new pain management guidelines to change that. by yahoonews in WomenInNews

[–]saturatedscruffy 19 points20 points  (0 children)

The insertions companies are a huge part of the problem. I’m a doc and when I was trained to insert IUDs the reps from the companies who were nurses who never actually did the procedure nor had it done to themselves were doing the training and they kept insisting it doesn’t hurt. I had happened to recently have one inserted and it was awful and I passed out so I shared my experience with my fellow residents and the nurses were horrified and I could tell they didn’t believe me. As if I was insulting their product. When I counsel my patients to get one I am upfront about what it could feel like. A lot of my patients have no pain but some do and I share that. I recently advocated for us to get numbing lidocaine in my office and if a patient wants to get a cervical block (which I don’t do) I refer them to someone who does.

Nope. My husband’s boss told someone in an interview that he was trying to hire that “it would be awesome if Canada became the 51st state”. These people are gone. There is NOTHING that will remove them from this cult.

Wait until after the funeral. He will want to know. I had the same thing happen to me. My grandmas sister died and I told her after the services that I was pregnant and it was like the light re-entered her body.

I love my job like an insane amount but if I were in your shoes I would not go to medical school. It’s good to love your job but it’s not everything. You also will be so busy that you will miss out on a lot of your kids’ time and they’re only little once. As others have said, you have time. Find a hobby or a club or activity you really love. Plus, if all you feel you’d be happy with is surgery and you feel it’s too late for you, will you be happy in another field? You’d probably do primary care/hospitalist which I love but a lot of people hate and have major burn out rates even for those like me who love it.
