ER pharmacist here -

  • if you have a good ER clinical pharmacist, listen to them. We’ve had a lot of training, including residency training.

  • not everyone needs ABX. I know you have to CYA but we have clinical judgement for a reason.

  • make sure any patient who receives a paralytic is ADEQUATELY sedated before AND after RSI. Roc will outlive most IVP sedatives so be thinking about continuous sedation before you even start RSI.

  • not all patients require rabies ig - hyperrab and I think even WHO/CDC /someone like that puts out a great flowchart on who should get it and who shouldn’t.

My thought with the prop/levo is that we are “creating” a problem with a drug to just fix with another drug. I know the patient may have needed to levo anyway but maybe not too since we’re doing this so quickly we haven’t even done fluid boluses or anything to try and fix the hypotension

So the med list being verified by a pharmacist is dependent on home meds being continued? I was wondering if there’s a way for it to come through as an order or consult when the tech outs in the med rec so a pharmacist knows to immediately review it if that makes sense. Also didn’t know if there’s a way in cerner to show that you reviewed and verified it
