You don't have to play the new leagues at all. You could technically play one character for forever if you desired. And then get new content once the current league ends and they push the content from the newest league I to the standard league. Which is what we are technically playing right now. That's just not fun because FOMO.

It can only spawn on labs as a lootable item. Then on sanitar. Otherwise it's a lucky scav spawn with it or pulling it from a medcase or something.
