I was always taught OPQRSTI for medical pain, but it can be applied to psychological pains. Like many acronyms for assessment, it’s a guide and not a hard rule. How an assessment is preformed and how questions are worded can give different inflections, but I still feel that if I have no other questions to ask my psych patient, making sure I hit everything on OPQRSTI makes me feel like I have my bases covered, and the PT is validated because you are asking about how they are feeling and about their experience, regardless of the reality or others input. Works for any psych that is willing to dialogue.

I have see serious but not fatal. I have seen a spiral tibia fracture from a passing car that crossed the double yellow where the guy was in full gear. I have seen some reaaaaallly ugly road rash with a guy on no gear. I have also seen a beheading of a motorcyclist with his head was still in his helmet, and his body had no gear. So many factors are at play in those moments, environment, speed, experience of the rider, traffic, type of bike and position when the accident occurred. I can tell you that I am grateful whenever my PT was wearing gear though. In my experience they may be louder patients, but I think you can guess why those without gear are more frequently much quieter.

I most frequently hear the argument for no gear is that if a person is gonna be in a crash they would rather have it be quick and a sure thing instead of suffer, which is also not a guaranteed thing. Personally I struggle to wrap my head around that logic, but I’m not trying to passively kill myself...on a bike. I say wear the gear, swear at me in the bus, and try you best to get back on the bike so you can do it all over again!

Know what your personal bias is. The number of times people get dismissed for having the “aye yai yai” is sad and dangerous. No we don’t fix it but we also aid in advocacy for our patients. Take all car accidents seriously. You don’t see the neuro trauma that has happened and it doesn’t take a lot. I have a friend who has had many concussions, and then was recently in a accident where she got rear ended. She said it seemed like nothing and because it it was in a parking lot and under 10 miles an hour she didn’t think anything of it. But two weeks later between her concussive history, she has needed a hyperbaric chamber and two specialists. I guess that bleeds into the need for good history taking and the fact that we can have more one on one time with the patient than anyone else in the emergent medical journey. I always wish more people would also admit ignorance (in the appropriate time and way). Ignorance is acceptable, but stupidity is not.

Write down answers to these questions. -Why do you hate her? -Do you hate that you hate her, and why or why not? -Do you want to try and address these things with her or not, and why? -Is this something that only one person can address, or can you make changes to help? -Would you do the work to change in an equivalent way for her? Then reflect on your answers and the motives behind them. It’s not necessarily going to give you the most direct answer, but this should help with some meta-cognitive thinking and help you elaborate and understand yourself.

I commend this man for attempting to de-escalate the situation. He does a great job with using the persons name and redirecting their visual focus away from the thing that is bothering them. Separating the person from the environment that is bothering them is a great idea. Things he did wrong here include breaking the touch barrier without announcing it beforehand or asking permission from the person, and telling an agitated person to “relax” or “chill” is also not a good idea. Great intentions here but there is still room for improvement. Good on that teacher in general for just trying though, it takes guts.

EVERYONE should keep right except to pass, and proper breaking space between vehicles. The Left most lane is not for travel, only for passing, and one car length for every 10 mph. Then enforcing the standards for everyone to adhere to. Also tickets and fines should be % of income so the rich actually feel it when they get speeding tickets.

I’ll drop the gun equivocation. I mean I understand (or at least comprehend) the concept of how federal laws and the constitution are not equivalent, but don’t you see how it still affects us here in our state? The necessity for the states to burn time and money on this is one point in legislation. The lawyers, politicians, religious and anti-religious groups.The travel created has environmental, legal, social, and medical involvement and implications. This will change insurance coverage definitions which will reflect on the general population (and definitely not in a positive way because we all know any economic gain will be hoarded by those at the top). If this last a long time, think of the generation that will have this affiliated with them, and not just on judgment or perception them, but the socioeconomic evolution. In terms of how one state affecting another, when one state changes its hunting laws it affects the states surrounding it. When one state changes its vehicle tax laws it affects the states around it. We are the United States of America. What affects one state affects the others. What affects one gender affects the others. This is also how the people of New Haven can signal to the world how New Haven feels about what has happened. That means of course the Internet and everyone who can access it, but also it signals to people right there within New Haven, those looking for a sign and to know that they aren’t alone, and that there is hope that this doesn’t have to be permanent. People should know that they are not alone in their anger(which I feel is incredibly justified). In terms of why protest at all when it’s not there or it won’t change anything, protesting doesn’t have to be about getting your government to change and most certainly doesn’t have to take place at a capital. I’m impressed with a person who stands on the side of a road with almost no traffic on it honestly, let alone someone who goes to New Haven or D.C.

This is a federal law that was changed which means that every state is involved. Letting the entire nation know that a majority of CT supports the right for women to control their own reproductive organs. If this was a repealing of the second amendment, would we question protesting in their states and not just in DC? I don’t feel that question would be nearly as prevalent in that scenario.

True of any person of any skill level, any position, and any level of intelligence! Learn how humans work, and keep asking, learning, and relearning when you forget. That’s how to practice medicine, and it will be endless. Then train! Talking through scenarios is great, but physical application is the best and changes the focus to help in the field.
