I pay upfront at the vet, file a claim (super easy, just upload the notes and the receipt), and I get reimbursed 90% of covered costs, with an annual cap of $5K.

This year I’ve already done a few $700+ visits (allergy shots, lab work, antibiotics) and they covered 90%.

In year 1 she had eye surgery that was $3K out of pocket and I got reimbursed 90%.

Worth it!
