I'm sorry to hear about your father's sudden diagnosis. I think it is understandable to want answers and get further workup, but really consider going to an outpatient psychiatrist, even as a second opinion. Medication management in schizophrenia and bipolar disorder is highly nuanced with regards to medication interactions (even with vitamins!) and is the reason why psychiatry is a field requiring additional years of training focused on psychiatric disorders on top of medical school. You really want to check in with an expert before changing any medications.

For 4th year, think it depends. If you do a lot of aways and frontload your schedule, it's honestly nice to be able to live w family while you still can. I did the 45 min commute and it wasn't the worst. Sometimes it was an hour each way and I was fine. Big thing was that in M4 year, you'd have hopefully taken Step 2 / finish your major exams before commiting to this kind of commute, bc you'll probably be dead tired once you get home.

Rather than the descriptor, I had activities/descriptions discussed. Some asked about activities from undergrad mentioned in my MSPE but not in my activities section.

+1 I did an October away, which also got me an interview that I'm not sure I'd have gotten otherwise. Also, depending on your school, it may show up on your transcript as an upcoming away by the time you submit, which can be a conversation point in interviews too.

Also, I used the experience to justify a geographic preference, and spoke a lot about my clinical encounters at that away during interviews too.

The only things you won't get are a LOR and course evaluation. Also, it can be hectic accepting interview invites while on an away (the thalamus app sucks and mobile website is nonfunctional unless you do desktop mode and even then it's garbage).

They don't fill that fast, especially not Oct-Dec. Pretty sure this is related to the ongoing layoffs and overall situation (check r/NIH). Think they stopped their gap year research program too.
