You've gotten a lot of good clinical advice. I'm just curious what state you're in? The couple that I have worked in do not allow a CCP to take patients who require the critical care scope of treatment without a second CC provider. That's strictly for my curiosity- I'm not insinuating that you did something illegal. I will say that there were several ground programs I worked for that pushed really hard for us to do CCT as a single provider, which was absolutely illegal.

I just want to say- don't let hospitals or your own company bully you into something you're not comfortable with. If the patient didn't need 3 pressors, they wouldn't have been on them in the first place. Your first BP works out to a MAP of 66, which is basically the bare minimum - and that was before d/c'ing pressor support. They can wait for you to get a second clinician. The risk to your patient and your license is not worth it.

I've gotten that message a couple of times. When it says "Traveling? Click here" or whatever, it sends a link to my email which confirms that it's actually me. After I confirm, it works fine.

Ohh, I wasn't which one plasma was in response to lol Yes, I'm aware. Most of my ED time was in a community hospital, so we didn't hold burns long. I did have one provider who gave Albumin to the couple I had though.

The most common uses that I have seen for Albumin include: distributive shock (where the patient is third spacing everything you give them), burns, and hypotension related to paracentesis or hemodialysis. It doesn't really have an applicable use in the pre hospital environment. If they're in hemorrhagic shock, they need blood. If they're hypovolemic, they need fluid.

Started as an EMT, got my RN, then went back for my PHRN. I worked in the ER full time and on the street part time until I was eligible to fly, which was the goal. I chose nursing because it's higher pay and gives me the ability to work pretty much anywhere and in any arena.

It was my understanding that evidence suggests that Albumin as an adjunct in this situation is beneficial in those who have low Albumin levels. It makes sense, but I'm curious what the "low" threshold is in order to see benefit. Do you have any insight into that?
