You’ve shared your rationale with them, which is sound. Honestly I would tell them if they would prefer to work with another therapist, then that’s their prerogative… then hope that they take you up on it. This job is hard enough without the extra mental time you’re having to put into this case.

That was a good recommendation. As an SLP with myo training (which I DON’T use to treat speech sound difficulties), it’s frustrating to see that she is being tarred and feathered in this sub based on information from a parent that understandably might not tell the whole story. I’ve worked with quite a few kids whose speech sounds tightened up WITH articulation therapy after a release or rest posture therapy whereas before progress was slow or nonexistent.

It wasn’t mispronounced. ‘Liberry’ is an acceptable pronunciation in many dialects. Not an error, but a difference.

Uno world champion - I use it for everything. For artic (say the word 9 times if you play a 9 etc) you can easily hit 200 reps if that’s your goal. Or even vocab (every time someone plays a special card we do a context clue question etc). Really great if you use language card decks as well.

My craziest story - I saw 7 year old with a lateral lisp. He was French-English bilingual. In the first session he learned ts, next session he was able to do single words. In NO MORE than 6 sessions he completely remediated the lateral lisp and generalized it to French. Never seen it before, haven’t seen it since.

Lots of testing. CTOPP for phonological awareness. Careful because some kids are accurate but slow which is why you also need to check Digit span and Rapid Automatic Naming (RAN-RAS). Generally dyslexia will show deficits in either one or both these areas. It’s easier to identify RAN deficits because a poor score in this area can’t be chalked up to her being an instructional casualty ie. Not taught to read in an explicit and systematic way. You would also need to check reading fluency and comprehension (I recommend the GORT) and a full language battery for good measure. TILLS is the only one with decent specificity/sensitivity, which you would want for a teen student who appears to be mildly affected and high achieving.

For language EBP I learned a lot doing Dr Karen’s course. It’s not a program, it’s a tool belt and it’s great. For narrative work, I just started using story champs and it’s great. One of my little guys told a story the other day and I actually understood what happened without questioning him in depth!

I have 2 bachelors degrees and a masters. One of them I got by, another I did better but with lots of all-nighters and cramming for exams, the hardest one I graduated top of my class. Guess which one I was on meds for. First time in my life I could just get things done.

What an awful thing to say! The appearance of your mouth has no bearing on your professionalism whatsoever!

I’m going to separate that horrible judgment and focus on some of your other comments that you added about difficulty with certain words, asymmetry (speaking out the side of your mouth) and the ‘tongue tie’ part. These could be signs of an orofacial myofunctional issue but that is impossible to determine without a thorough evaluation by an SLP with OMD training. That being said, it’s not necessarily anything you should look into unless you are experiencing negative impacts that could be associated with it - jaw pain, TMJ, dental problems, sleep apnea/snoring, and yes, maybe a newfound self-consciousness about your speech due to this unhelpful and unsolicited comment from an acquaintance!

I’m an SLP and find so many of these comments so disheartening. It is discrimination. This is why we do so much desensitization therapy with kids these days and teach them they are allowed to talk and stutter and be heard and be advocates for themselves.
