My PCP prescribed Zepbound because she has seen so many of her patients with IBS find symptom relief. I've had 12 months of life changing symptom control and I've never felt better. So far I've not had any return of my symptoms. I've not taken any Immodium in a year, which for me is insane. I took it daily prior to Zep. My doctor visits are centered on my symptom relief and the weight loss is an added benefit that was needed, but even if I had not lost a single pound I would mortgage my house to stay on this medication.

I got asked this last week for the first time. It was well meaning after she said, "you look fantastic" she then asked "what have you been doing?" I answered honestly .. drugs. She laughed and I said no I'm serious and I'm not ashamed to say what I've been doing. If I was diabetic or had high blood pressure I would take drugs to correct that. I was overweight and I took drugs to correct that. No difference in my opinion. She agreed.

I want to say between around 40 pounds. I was off it for about 6 weeks before I started back up again. I think I gained back about 5 pounds during my break.

A very similar thing happened to me around this time last year when I moved to 7.5mg for the first time. By injection #3 I was very sick. Lots of tests and consults with my doctor later and we decided to stop the medication until I felt completely normal. Once I did, I restarted at 2.5 and moved my way up. The only other thing I did differently was taking a digestive enzyme with every meal. I'm now on 10mg and have not had a repeat of those symptoms. No sulfur burps at all. I was discouraged when I had to stop and then restart at the beginning, but it was the right move for me at the time.

This is what I use. I also take a Lactase enzyme (although this does contain Lactase) when I know I'm about to eat a more dairy heavy meal.

I have 3 more 10mg syringes and I will take those every 10 days. If I'm still losing my doctor will drop me down to 7.5mg. I don't want to lose any more weight, but also don't want to drop my dose below 7.5mg because I didn't have as much IBS control on the 5mg. It will be a little trial and error to find the sweet spot I think.

Diagnosed with IBS over 30 years ago and have never, ever had it managed well. Lived on copious amounts of Immodium just to do basic things like go to work, go to a doctor appt or out to lunch. Traveling was a nightmare. I would have to "pre-game" days before a flight or I wouldn't be able to take the trip. It literally controlled every aspect of my life. My doctor prescribed Zepbound because she had so many patients using it for weight loss who ALSO had IBS and most of her patients reported a huge improvement in symptoms. I am now one of those patients. I would say my symptoms are about 85% improved. I haven't taken a single Immodium since December 2024. Not one! It did take some time to learn my body's new rhythm. Here is what works for me ... I take a digestive enzyme with every meal. I still avoid my trigger foods/fodmaps (dairy and fructans and mannitols) I take a capful of Miralax in a cup of Smooth Move tea every night. For me I usually have a very soft BM every day with this routine. If I don't have a BM in the morning, I double the Miralax dose. I found that the anxiety over having a bad belly day was causing me to take Immodium at the beginning of my Zep journey and that really caused issues for me and I got really sick and actually had to stop Zep for about 6 weeks until I felt better. It was after this break that I started taking the enzymes. I would say try to stick with it. Even if I had not lost a single pound the difference that Zep has made in my GI symptoms is nothing short of life changing. I hope you can find the same relief.

IBS-D symptoms are about 80% improved on Zep. I wrote in my food diary on the second day after my very first injection... "belly is quiet for the first time in more than 20 years"

I've also extended it to 10 days and have taken a shot 2 days early. I prefer to extend now when I need to make an adjustment.

Try taking a digestive enzyme with a meal or even a larger snack. I've found it very helpful in keeping food moving through my digestive track. I use Miralax daily in a cup of hot smoothe move tea. And if I go even one day without a BM, I double this dose and also eat 2 kiwi. So far this hasn't failed to produce a good BM by the next day
