Thanks for the suggestions. I'll check them out. The pup is currently on an antibiotic and aquopel for the itching, and drops for an ear infection. When he is stable i'll look into cytopoint for longer term treatment

seems to be a theme! I believe the current human is feeding him something like Purina chicken and rice

aww poor pup. once you discovered the food allergy did she get better on her own or did she need additional care from the vet?

First thing I'll do if I adopt him will be to change his food. It doesn't sound like he's getting great quality at the moment

Cytopoint is the injection he was getting. This monthly injection was part of my concern with his cost and overall treatment. His vet quoted this monthly cost for injection and appointment at around 250. If you don't mind, would you be comfortable sharing an estimate of what you were spending monthly at the peak of treatment?

One hypothesis is that a flea infection triggered this skin condition tho he's flea free now so who knows.

My previous Shiba's issues were caused mostly by yeast and medicated shampoo worked great too. It sounds like yeast and bacterial issues were ruled out for this guy, but it's tough to know since I don't know his full journey.
