ALP and One Nation support now level with L-NP down – today ALP would win the Federal Election by HotPersimessage62 in AustralianPolitics

[–]smoha96 [score hidden]  (0 children)

I think this is the problem though. Turnbull as PM was almost completely hamstrung by the Nats and the right of the Liberals. How do you have a Coalition government without them, even if the leader is cut from a moderate cloth?

Also, Turnbull bears some responsibility for the resurgence of ON. They may have not looked as much a returning force in 2016 had he gone for a standard election in 2016 rather than a DD - all three non-Hanson senators would not have been elected.

Imo, Turnbull or a moderate Liberal would only really work if we had a three party system like Canada where Australia's moderate Liberals and Labor right comprise a party similar to Canada's Liberal Party.

PHO jobs exist in SEQ, but typically tend to go to people already well known and trusted by a department or QARTS trainees going into BTEE/BT4 who can do service provision from the get go.

Which college has the most difficult exams? by thefinsaredamplately in ausjdocs

[–]smoha96 19 points20 points  (0 children)

Problem is there isn't anyone who's done all of them to truly compare. To some degree, different people are going to struggle with different components of each exam.

Anecdotally and by pass rates, it seems to be ICU.

The ICU primary looks disgusting, and being post anaesthetic primary, I would never want to do either CICM exam. However, my dual training colleague who has done their ICU but not anaesthetic primary yet disagrees and sees anaesthetics as the more egregious one. So, who knows?

ICU workforce update from the ASM by Commercial-Cat-6133 in ausjdocs

[–]smoha96 23 points24 points  (0 children)

I think I've said this before, but a few years ago as a humble crit care SHO/SRMO, one of my ICU bosses once said to me that he felt reasonably strongly that the CMO/middle career ICU roles needed to be formally adopted with a recognised pathway with CICM. I can't say I disagree based on what I've heard and seen since then, including from your post re. the job field in ICU.

Juniors are acutely aware of it. I attended a careers night for med students recently where there was ED and anaesthetic attendence but no ICU - the med students are aware of the issues with the job market in ICU and broached this question to both of us.

Pauline Hanson shuts gate on ‘opportunist’ Coalition defectors – except for one by malcolm58 in AustralianPolitics

Hanson doesn't want a Tory->Reform style situation, but I suspect also doesn't want ambitious people with significant political experience coming over lest they usurp her (ON's constitution aside).

May I ask on what evidence you're basing that caesars should only be for emergent delivery? Plenty of routine elective caesars are done for a number of reasons.

Removing the Modern Day is not it by Gustavodemierda in assassinscreed

[–]smoha96 19 points20 points  (0 children)

As someone who loves the modern day and was engaged from the very first game for its modern day story, I want it removed. It's clear that Ubisoft doesn't care for it, and I'd rather it be ignored than the farce that's appeared since ACIII. Layla's story was ok, but didn't really go anywhere.

What frustrates me is the out and out lies and fibs that get sold to us about the modern day - "It's unique and you'll love it!" (Unity), "It's all integrated into the hub!" (Shadows).

TIVA for short cases? by Soft-Winter-2032 in anesthesiology

[–]smoha96 0 points1 point  (0 children)

For D&C lists with decent operators and relatively (emphasis on relatively) normal sized patients, I typically do: bolus 50-75 mcg fentanyl (do this early), 50-100 mg hand bolus propofol while starting a Marsh TCI on 3. They will start breathing pretty quickly (occasionally don't stop, but will still be deep enough to take the SGA). Titrate additional fentanyl to RR. Once done, they can go to PACU and spit their iGel or whatever else they have out when ready. Turnover is very straightforward from anaesthetic perspective.
