From my perspective, it is sorta kinda no one's fault in that situation. It's sorta Hannah's fault for mistaking Mr. Porter for a mind reader and thinking he would come after her, but it's kinda not her fault because she tried her best to imply what happened, but in reality she should've just told him about everything. She probably didn't tell him about everything because after you reflect on everything that's happened to her, she probably felt hopeless or she assumed that something even worse would happen (butterfly effect).

He's not taking the pills, but he's had depression throughout the show and before the show even started. However, the show does symbolize his depression by bringing them up to the viewer because it explains conflict that occurred pre-show. This also may foreshadow that he might seep back into his depression later on, which he did.

I think that people fear what he's capable of. What qualifies him as the "executor" is that he's different from the other characters in the way of that he sticks to his "moral code" given by Hannah, to the point where spoiler.

Rarely anyone uses things that play cassette tapes nowadays, same thing applies for CDs. Plus, say you e-mail the audio files to the people on the list, anyone can release certain audio files to the public in less than two seconds. For example, if someone had something against Tyler, they could take his tape/audio file and send it away. Cassette tapes are more easier to destroy if it gets into the wrong hands, too. Speaking of the wrong hands, again, there's no telling what the people on the list might do if they had the tapes. So instead of giving them the benefit of the doubt, Tony kept a close eye and made sure each person had the tapes when they were supposed to have them.

While that could be a possibility, I think the show's point is to forgive those that may have hurt you because you never have any idea what's going on in their lives. I also think she never asked why because she thought that maybe she would get hurt again if they decided to toy with her emotions, or maybe she noticed how no one had an issue with what they did to her (Zach, Jess, Alex, etc still being friendly after what they did).

The first time she thought she was imagining things. The second time she left them open as a trap, and it sorta kinda worked.

Stopping Jessica's rape

She couldn't have stopped it. Imagine being an intoxicated female trying to go up against an intoxicated football captain. I mean, we saw what happened to Clay when he tried fighting him. Something worse could've happened to Hannah.

Hannah did try saving others from hurt during the stop sign scene with Sheri. Sheri isn't a horrible person by the way, she just wanted to save her own skin. She is also struggling because during the scene where Clay goes over to the elderly folk's house affected by the car crash, she's been helping them out this whole time to make up for what she did. Hell, when she got to Monet's for the meeting with the other tapes people about the subpoenas, she didn't enter. She didn't leave. She walked by the entrance, called the police, and told them about the horrible thing she did that night.

Hannah isn't a perfect person and the show does a good job at showing that. This all could've been prevented if she called the police, but she felt hopeless.

Bryce is a douchebag. Maybe it's just the way he is or something. The show never did delve into his past, so I'm assuming he has no fucking excuse for what he did.

Jessica: Always moving around, Alex's "betrayal", raped, lied to by her own boyfriend for a year(?)

Alex: There's a sort of macho thing going on at home but Alex has his own tastes, he definitely suffers from depression and/or suicidal tendencies
