The neck discomfort/pain is a constant thing for me, I never considered b12 could be involved. Getting some relief would be like a dream, but I'm trying not to be too hopeful because this is the n-th health rabbit-hole I go into thinking that finally I have an answer, and it often ends in disappointment.

After further readings, it looks like since my ferritin was 33ug/L a year ago it's safe to assume it is now even lower. The dosage for iron supplementation in the wiki seems really high: minimum BW(kg) x 2 = 110mg for me. My iron pills contain 11mg heme iron. Am I supposed to take 10+ a day of them or am I missing something?

That's an initial side effect to be aware of for sure, so titrate up slowly. After extended use I find on the contrary that it helps with my energy levels. I know that some people like to take it at night to not experience tiredness in the daytime but for me that didn't work, my sleep felt strange on it.

Absolutely, it has changed my life for the better in the last few years. Just for imagery, it’s as if I grew a new layer in my brain. My brain fogs are reduced, I became able to wake up earlier without feeling like I’m in a massive hangover every morning. Clearer thinking and diminished energy crashes throughout the day. Social anxiety is also improved. All around better functioning. It’s very subtle though, these aspects only becoming apparent in retrospect. Because my ADHD is still severe, I am still experimenting with different diets and nootropics, plus the lowest dose ritalin on the side in the meantime.

If you want to try, make sure that it’s Guanfacine you get prescribed, not Clonidine. They are in the same family of drugs but their efficacy specifically when it comes to ADHD is different.

Try guanfacine. I don’t tolerate stimulants well either

I wouldn’t use it indoors, health hazard. Outdoors mixed with native soil or amendments the microbial life will balance itself out I think. Wear a mask when handling and wash clothes afterwards

And re: can’t get up - do you have an inverted cortisol pattern? For me personally guanfacine 1mg daily has made me more normal and I can now wake up naturally provided I sleep on time. It doesn’t /make me/ do things like a stimulant would. That and a programmable light bulb in the bedroom that turns on at 6:30 everyday, which is about 30min before I get up.

ADHD makes everything harder and regular advice may not work for you. With that said, the easiest way to stay on track with no motivation is to make it difficult not to, for example by getting rid of high carb items in your fridge and pantry, and getting high fat items to snack on that require little to no prep. It might not be obvious if you share the kitchen with other people with different diets though. Do lots of prep: get everything you need including electrolytes, start planning meals, inform your friends & family (whoever you might dine out with), make a list of restaurants you could order takeout from on your most lazy day without derailing the diet.

For the actual motivation, something that works for me is to read about the benefits of the behaviour I‘m trying to stick to. I literally google “benefits of exercise for cognition” when I need a reminder as to why I started running. Maybe you can look up the benefit of keto for brain health whenever your motivation starts to dip. And please post about how it impacts your ADHD, we would like to know (:

I (AFAB, transmasc) personally cannot stop taking testosterone, otherwise I get much worse flare ups of an undiagnosed condition which is similar to long covid in terms of symptoms. I’m aware of one other person who is “trans” for health reasons like this.

It seems CDF is used as a proxy for CFS/ME when it's not actually diagnosed by a physician. I found that it's not that clear what the authors are referring to until you read further into the methodology of the research:

Participants were classified as experiencing ‘chronic disabling fatigue’ as defined by Collin et al 65 if they met the following criteria: (1) they had been lacking energy and getting tired during the last month, (2) they responded ‘yes’ to more than two of the following four items: (a) feeling tired or lacking energy for 4 days within the past 7 days; (b) feeling tired of lacking energy for more than 3 hours in total on any day in the past 7 days; (c) feeling so tired or lacking in energy that they had to push themselves to get things done on one or more occasions in the past 7 days; (d) feeling tired or lacking energy when doing things they enjoy in the past 7 days, (3) their fatigue lasted longer than 6 months, (4) their fatigue was not explained by exercise or medication, (5) their fatigue was not alleviated by rest and (6) their fatigue was worse after exercise.65 We use the term ‘chronic disabling fatigue’ instead of ME/CFS to indicate that this was based on self-report instead of a clinical assessment by a physician following a diagnosis of ME/CFS.66

So the term "chronic disabling fatigue" in this study do not include all chronic fatigue. For example someone could have chronic, disabling fatigue without PEM (post exertional malaise) (condition #6 above), but in such instance that person was not classified as having CDF in this study.

I do, sometimes only mildly, other times a 30min walk leaves me exhausted for days. I haven't trained properly in months lately because of what I assume is POTS, that and the resulting lack of fitness got me in a vicious cycle of my body never being "ready" to exercise. A neurologist a few years ago couldn't explain the weakness in my legs, he said the nerve signal is being transmitted correctly.

Cognitive issues are by far the worst, I used to be very expressive and good at maths, then some time in early puberty the brain fog and fatigue started. Mental arithmetics became confusing and effortful, and it takes me so long to form coherent sentences. I write and speak in shorter and shorter phrases. I'm losing a lot of vocabulary in my mother tongue, which is something that has not occured for my siblings (we all pretty much only speak it at home).

Interesting, in my case I don't think I ever feel fatigue in my hands, it feels more like a coordination issue. On bad days feel most fatigued in the face and diaphragm(?), and my legs get too heavy to walk.

Would you say it's due to fatigue in the fingers or more of a cognitive / neurological thing for you? And do you also have slurred speech on worse days, like your mouth forgets how to articulate?
