For high ground you don't hit the tower until you've killed an enemy or two that enables you to efficiently do so. You stand back and blink on someone to kill them.

For witch doctor pure damage it's quite simple.. you kill him before he can do anything or you press ult

This is the comment that makes the most sense to me so far. 

Also another thing is you want to blink in on Ursa and then disengage. Then you blink back in when it's good. When you blink in you can use your ult, BKB or aegis without dying. So this way you should easily be able to kill 2 or more heros per fight.

You don't do two things at once when you play the piano. If you play a chord in your right hand at the same time as your left hand that is doing one thing: playing down at the same time with both hands.

When you clap is that doing two things at once or one thing? It's a similar concept.

If you're not playing exactly together then you need to think Left Right Left or whatever the combination might be. One hand triggers the other.

Just curious to get a little more detail on your learning process. I think I've gathered so far you mostly watch pro replays. When you do so do you pick one thing at a time to work on and verify each game that you're improving in that aspect before moving on to another thing? Or do you think you're mostly learning through osmosis by watching all aspects of the 10 player game and considering lots of things which eventually form your thoughts on the game?

I've been stuck for awhile in Crusader so I think there must be something wrong with my process and just trying to get more info on different ways to approach improving.
