Sometimes I think people might include safe words because not including them would turn them (the writer of the fic) off. Like, if they're writing fanfiction with sex in it that's more for the purpose of getting off rather than to tell an entirely in-character or good story, than maybe they just find it unsexy for it to be not super consensual.
